【摘要】：目的探討老年腰椎滑脫患者微創(chuàng)手術(shù)和內(nèi)固定治療的臨床研究。方法選取2013年5月至2015年4月在該院接受治療的腰椎滑脫患者60例,隨機(jī)分為觀察組和對照組各30例,觀察組腰椎內(nèi)固定治療采取微創(chuàng)手術(shù)進(jìn)行,對照組采用常規(guī)手術(shù)方法,比較兩組患者手術(shù)時間、術(shù)中出血量、日本骨科學(xué)會(JOA)評分、Oswestry功能障礙(ODI)指數(shù)以及視覺模擬評分法(VAS)評分情況、術(shù)后影像學(xué)改變情況(椎間隙高度、融合率)和兩組患者對療效的滿意度情況。結(jié)果治療后,觀察組手術(shù)時間、術(shù)中出血量明顯低于對照組,射線暴露時間明顯高于對照組,JOA評分高于對照組(P0.05)。治療后,兩組ODI指數(shù)以及VAS評分均下降,且差異均有統(tǒng)計學(xué)意義(P0.05)。兩組術(shù)后1個月的矯正率有所下降,且兩組比較差異有統(tǒng)計學(xué)意義(P0.05)。兩組術(shù)后1個月的椎間隙高度變化以及骨性融合均升高,組間比較差異有統(tǒng)計學(xué)意義(P0.05)。觀察組治療1個月后滿意度高于對照組(P0.05)。結(jié)論微創(chuàng)手術(shù)聯(lián)合內(nèi)固定治療腰椎滑脫患者手術(shù)時間短、術(shù)中出血量少、提高患者骨性融合率,減少患者的疼痛感受,有效提高治療效果,并且治療后患者的滿意度比較高。
[Abstract]:Objective to investigate the clinical study of minimally invasive surgery and internal fixation in elderly patients with lumbar spondylolisthesis. Methods 60 cases of lumbar spondylolisthesis treated in our hospital from May 2013 to April 2015 were selected. Two groups were randomly divided into observation group and control group (30 cases each). The patients in the observation group were treated with minimally invasive surgery, and the patients in the control group were treated with conventional operation methods. The time of operation and the amount of intraoperative bleeding were compared between the two groups. Japanese Orthopedic Association (JOAA) score Oswestry dysfunction (Oswestry) index and visual analogue score (VAS) score, postoperative imaging changes (intervertebral space height). Results after treatment, the operation time and intraoperative blood loss in the observation group were significantly lower than those in the control group, and the radiation exposure time was significantly higher than that in the control group. The JOA score was higher than that in the control group (P 0.05). After treatment, the ODI index and VAS score of the two groups decreased. The difference was statistically significant (P 0.05). The correction rate of the two groups decreased 1 month after operation. The difference between the two groups was statistically significant (P 0.05). The height of intervertebral space and bone fusion increased 1 month after operation in both groups. There was significant difference between the two groups (P 0.05). The satisfaction of the observation group was higher than that of the control group after one month. Conclusion the operation time of the patients with lumbar spondylolisthesis treated by minimally invasive surgery combined with internal fixation is shorter than that of the control group. The amount of intraoperative bleeding was less, the rate of bone fusion was improved, the pain feeling of patients was reduced, the effect of treatment was improved effectively, and the satisfaction of patients after treatment was higher.
