【摘要】：目的：比較胸腔鏡與傳統(tǒng)開胸兩種手術(shù)方式治療肺大皰合并自發(fā)性氣胸的療效。方法：通過內(nèi)蒙古包鋼醫(yī)院2001年3月至2014年10月收治的肺大皰合并自發(fā)性氣胸患者136例作為試驗(yàn)對象,其中胸腔鏡手術(shù)76例,傳統(tǒng)開胸手術(shù)60例,通過觀察其手術(shù)用時間、手術(shù)后胸引管留置時間、手術(shù)后抗生素使用天數(shù)、手術(shù)后住院時間、手術(shù)治療費(fèi)用、術(shù)后止痛藥物使用例數(shù)、術(shù)后并發(fā)癥發(fā)生例數(shù)、術(shù)后復(fù)發(fā)例數(shù)等指標(biāo)評價兩種手術(shù)方式的療效。結(jié)果：136例患者手術(shù)均取得成功,無圍術(shù)期死亡,兩組患者手術(shù)順利,均治愈出院。胸腔鏡組手術(shù)后切口疼痛感輕、手術(shù)用時間、術(shù)后胸引管留置及術(shù)后住院時間短,在術(shù)后抗生素及止痛藥物用量方面少優(yōu)于傳統(tǒng)開胸組(P0.05),而住院費(fèi)用傳統(tǒng)開胸組明顯低于胸腔鏡組(P0.05)。結(jié)論：兩種手術(shù)方式治療肺大皰合并自發(fā)性氣胸均安全并有效,胸腔鏡治療肺大皰合并自發(fā)性氣胸術(shù)后疼痛輕,抗生素及止痛藥物應(yīng)用少,傳統(tǒng)開胸組醫(yī)療費(fèi)用較經(jīng)濟(jì)。
[Abstract]:Objective: To compare the efficacy of two types of surgical assisted thoracoscopic surgery and conventional thoracotomy in the treatment of pulmonary bullae with spontaneous pneumothorax. Methods: the Inner Mongolia Baotou hospital from March 2001 to October 2014 were pulmonary bullae with spontaneous pneumothorax in 136 patients as the experimental object, including 76 cases of thoracoscopic surgery and conventional thoracotomy in 60 cases, through the observation of its operation with time, operative chest drainage tube indwelling time, postoperative days of antibiotic use, hospitalization time after surgery, surgery costs, postoperative analgesic drug use cases, postoperative complication cases, postoperative recurrence cases to evaluate the curative effect of two surgical methods. Results: 136 cases were made success, no perioperative death, two patients were operated successfully, were cured. Thoracoscopic operative incision pain light, operation time, postoperative chest drainage tube indwelling and postoperative hospital stay in a short time, In terms of postoperative antibiotics and analgesic dosage less than traditional thoracotomy group (P0.05), but the hospitalization costs of traditional thoracotomy group were significantly lower than those in the VATS group (P0.05). Conclusion: two kinds of surgical treatment of pulmonary bullae with spontaneous pneumothorax were safe and effective, thoracoscopic treatment of pulmonary bullae with spontaneous pneumothorax in postoperative pain, antibiotics and analgesics, traditional thoracotomy group medical cost is economic.
