可吸收縫線材料的可降解與臨床應(yīng)用

【摘要】:背景:由于可吸收縫合線具有良好的生物相容性及機械強度,在臨床被廣泛應(yīng)用。目的:探討不同可吸收性縫線的體外降解性能,分析可吸收縫合線臨床應(yīng)用中的優(yōu)勢及其體外降解行為的影響因素。方法:通過檢索近年來文章內(nèi)容與可吸收縫線材料降解性能和應(yīng)用相關(guān)的文獻(xiàn),從可吸收縫線材料的性能及降解行為影響因素、可吸收與不可吸收縫合線臨床應(yīng)用比較等對可吸收縫線材料相關(guān)研究成果作回顧性分析,為臨床提供理論依據(jù)。結(jié)果與結(jié)論:可吸收縫合線體可被機體完全降解吸收,無不良反應(yīng),創(chuàng)口愈合后不留瘢痕,克服了不可吸收縫合線不能在體內(nèi)分解的缺點,其降解產(chǎn)物的體內(nèi)吸收主要有2個途徑:一為體內(nèi)巨細(xì)胞與吞噬細(xì)胞吞噬,另一途徑為降解產(chǎn)物進(jìn)入人的體液與血液,如乳酸通過體循環(huán)被吸收排泄。人體內(nèi)有一個極其復(fù)雜的生理環(huán)境,存在著影響材料性能的各種因素。加之材料的種類不同,它們在體內(nèi)的降解行為也不盡相同,因此需要在一定范圍內(nèi)設(shè)計合成和加工出具有特殊性能的可降解高分子材料滿足臨床需要。
[Abstract]:BACKGROUND: The absorbable suture has good biocompatibility and mechanical strength and is widely used in clinic. Objective: To study the in vitro degradation of different absorbable sutures, and to analyze the advantages of absorbable suture in the clinical application and the influencing factors of in vitro degradation behavior. Methods: The properties of the suture material and the influencing factors of the degradation behavior of the absorbable suture material were analyzed by searching the literature related to the degradation performance and application of the absorbable suture material in recent years. The clinical application of absorbable and non-absorbable suture materials is analyzed retrospectively, and the theoretical basis for clinical application is provided. Results and Conclusion: The absorbable suture body can be completely degraded and absorbed by the body, no adverse reaction, no scar after the wound healing, and the defect that the non-absorbable suture cannot be decomposed in the body is overcome, and the in vivo absorption of the degradation product is mainly 2 routes: One is the in vivo giant cell and phagocyte phagocytosis, and the other route is to degrade the body fluid of the product into the human body and the blood, such as the lactic acid is absorbed and discharged through the body circulation. There is an extremely complex physiological environment in the human body, and there is a variety of factors that affect the performance of the material. In addition to the different kinds of materials, the degradation behavior in the body is different, so it is necessary to design and synthesize and process the degradable high-molecular material with special performance in a certain range to meet the clinical needs.
