【摘要】：選擇山東省立醫(yī)院自2005年1月至2012年4月間因各種緩慢型心律失常和快速型心律失常植入永久心臟起搏器(包括ICD)的患者共計494名。其中男性患者247名,女性患者247名,男：女=1：1。年齡最大者89歲,最小者6歲,平均年齡65歲。494名患者中：病態(tài)竇房結(jié)綜合征者387例,Ⅲ度房室傳導(dǎo)阻滯者103例；室速、室顫者2例,心衰2例。其中植入單腔起搏器共192例,雙腔起搏器298例,三腔起搏器2例,ICD2例。隨訪494名患者,發(fā)生并發(fā)癥共計31例。分別為囊袋感染12例,占2.4%；囊袋血腫(積血)4例,占0.8%；起搏器綜合征4例,占0.8%；電極導(dǎo)線脫位4例,占0.8%；電極導(dǎo)線斷裂2例,占0.4%；起搏器介導(dǎo)的心動過速1例,占0.4%；誤穿鎖骨下動脈2例,占0.4%。電池提前耗竭1例,占0.2%；心肌穿孔1例,占0.2%。分析并發(fā)癥出現(xiàn)的原因,并給予積極妥善的處理,旨在提高醫(yī)務(wù)人員對并發(fā)癥的重視,及時發(fā)現(xiàn),及時處理,并盡量減少其發(fā)生率。
[Abstract]:A total of 494 patients with permanent cardiac pacemaker (including ICD) implanted in Shandong Provincial Hospital from January 2005 to April 2012 because of various bradyarrhythmia and tachyarrhythmia were selected. Of these, 247 were male and 247 female, male: female 1: 1. The oldest was 89 years old, the youngest was 6 years old, the average age was 65 years. 494 patients included 387 cases of sick sinus node syndrome, 103 cases of third degree atrioventricular block, 2 cases of ventricular tachycardia, 2 cases of ventricular fibrillation and 2 cases of heart failure. Among them, 192 cases were implanted with single chamber pacemaker, 298 cases with double chamber pacemaker and 2 cases with ICD2 with triple chamber pacemaker. A total of 31 patients were followed up with complications. There were 12 cases of pouch infection (2.4%), 4 cases of pouch hematoma (hemorrhage) (0.8%), 4 cases of pacemaker syndrome (0.8%), 4 cases of dislocation of electrode wire (0.8%), 2 cases of electrode wire rupture (0.4%), and 1 case of pacemaker mediated tachycardia. The subclavian artery was perforated by mistake in 2 cases (0.4%). Early depletion of the battery was found in 1 case (0.2%), myocardial perforation in 1 case (0.2%). To analyze the causes of complications and to deal with them actively and properly, the aim is to improve the medical staff's attention to the complications, to find them in time, to deal with them in time, and to reduce the incidence as far as possible.
