【摘要】：目的利用三維有限元分析方法,研究髖臼發(fā)育不良患者加深磨銼假體置入部位的髖臼骨床、使臼杯假體內(nèi)移置入對髖臼假體-骨界面間的應力分布情況的影響。方法選取髖關(guān)節(jié)發(fā)育不良患者的骨盆為實驗對象,螺旋CT做全長連續(xù)掃描,然后利用計算機仿真技術(shù)對CT圖像進行三維重建,建立髖關(guān)節(jié)發(fā)育不良骨盆模型。在計算機環(huán)境中對重建模型進行髖臼磨銼,臼杯假體不同程度內(nèi)移植入的模擬手術(shù),利用有限元分析軟件SolidWorks對重建模型進行有限元網(wǎng)格化及力學分析。結(jié)果臼杯假體的內(nèi)移使臼杯上緣處獲得骨床包容,臼杯接觸面積更大,單位面積所受應力相對較少。隨內(nèi)移程度加大,髖臼內(nèi)壁骨質(zhì)突破更為明顯,使髖臼頂部應力集中及其余部分應力不均愈趨明顯。結(jié)論髖臼加深磨銼臼杯內(nèi)移植入有助于改善髖關(guān)節(jié)發(fā)育不良病例行髖關(guān)節(jié)置換臼杯的骨床包容,但隨內(nèi)移程度的加大,髖臼內(nèi)壁突破,髖臼面的應力集中,髖臼的應力分布情況明顯改變,應盡量減少穿破的程度。
[Abstract]:Objective to study the effect of acetabular implant placement on the stress distribution between acetabular prosthesis and bone interface in patients with acetabular dysplasia by using three-dimensional finite element analysis (FEM). Methods the pelvis of the patients with hip dysplasia was selected as the experimental object. Spiral CT scan was performed continuously. Then the CT images were reconstructed by computer simulation technique to establish the pelvic model of hip dysplasia. In the computer environment, the reconstruction model was simulated with acetabular burrs and cup implants in different degrees. The finite element analysis software SolidWorks was used to mesh the reconstructed model and analyze the mechanics of the reconstructed model. Results the inner movement of the cup prosthesis made the upper edge of the cup contain the bone bed, the contact area of the cup was larger, and the stress per unit area was relatively less. With the increase of the degree of internal displacement, the bone breakthrough of the acetabular inner wall became more obvious, and the stress concentration at the top of the acetabular and the uneven stress in the rest of the acetabulum became more and more obvious. Conclusion Transplantation of acetabular burrs in acetabular cup is helpful to improve the bone bed inclusion of acetabular replacement cup in patients with dysplasia of the hip joint. However, with the increase of the degree of internal displacement, the acetabular inner wall breaks through, and the stress concentration on the acetabular surface is concentrated. The stress distribution of acetabulum is obviously changed and the degree of breaking should be reduced as far as possible.
