保留軟骨的去細(xì)胞全喉支架制備方法及在喉軟骨缺損中的修復(fù)效果

【摘要】:目的探討保留軟骨的去細(xì)胞全喉支架的制備方法及在大鼠喉軟骨缺損中的修復(fù)效果。方法取6只大鼠用于支架的制備,分離頸總動(dòng)脈,結(jié)扎分支,經(jīng)頸總動(dòng)脈灌注去污劑(SDS、Triton X-100、PBS離子型),3只大鼠去除喉黏膜、肌肉、韌帶的同時(shí),保留細(xì)胞外基質(zhì)和軟骨細(xì)胞制備保留軟骨的去細(xì)胞全喉支架,其余3只用于制備對(duì)照組支架。另取34只大鼠,建立0.5 cm×0.5 cm全層喉軟骨缺損模型,根據(jù)處理方法不同隨機(jī)分為對(duì)照組(n=17)和保留軟骨組(n=17),對(duì)照組植入未經(jīng)處理的異體喉骨支架,保留軟骨組植入保留軟骨的去細(xì)胞全喉支架,比較兩組的修復(fù)效果。結(jié)果 (1)兩種支架經(jīng)過(guò)脫細(xì)胞灌注處理后,全喉大體結(jié)構(gòu)完整,體積未發(fā)生明顯的變化;(2)保留軟骨的去細(xì)胞全喉支架未見(jiàn)表面細(xì)胞組織填充,可見(jiàn)膠原、細(xì)胞外基質(zhì)成分組成的腔隙,細(xì)胞之間結(jié)構(gòu)緊密,存在于會(huì)厭、聲帶、環(huán)狀軟骨等表面;對(duì)照組支架組織中的細(xì)胞成分被完全去除,但是依稀可見(jiàn)網(wǎng)狀連接,結(jié)構(gòu)之間不緊密;(3)免疫熒光結(jié)果顯示:保留軟骨的去細(xì)胞全喉支架除了有肌肉、軟骨細(xì)胞外,主要組織相容性復(fù)合體(MHC)-Ⅰ、MHC-Ⅱ染色均為陰性;對(duì)照組支架免疫熒光染色下MHC-Ⅰ、MHC-Ⅱ染色均為陽(yáng)性;(4)保留軟骨組大鼠修復(fù)6 w后缺損部位存在少許不成熟的軟骨細(xì)胞,極少的軟骨形成陷窩,表面存在纖維組織及炎癥細(xì)胞;修復(fù)12 w后缺損部位得到修復(fù),形成軟骨陷窩,炎癥反應(yīng)減少;對(duì)照組修復(fù)6 w后缺損部位周?chē)浌羌?xì)胞較少,缺損部位與周?chē)M織邊界明顯,存在大量炎癥細(xì)胞;修復(fù)12 w后存在少量纖維組織,存在炎癥因子。結(jié)論保留軟骨組植入保留軟骨的去細(xì)胞全喉支架在喉軟骨缺損中具有良好的修復(fù)效果。
[Abstract]:Objective to investigate the preparation method of chondrogenic acellular whole larynx scaffold and its effect in repairing rat laryngeal cartilage defect. Methods six rats were used in the preparation of stent, the common carotid artery was isolated, the branches were ligated, and three rats were perfused through the common carotid artery to remove the laryngeal mucosa, muscle and ligaments. Extracellular matrix (ECM) and chondrocytes were retained to prepare chondrogenic laryngeal scaffolds. Another 34 rats were selected to establish a 0.5 cm 脳 0.5 cm full-thickness laryngeal cartilage defect model. According to the different treatment methods, the rats were randomly divided into two groups: the control group (nong17) and the cartilage preserving group (nong17). The untreated laryngeal bone scaffold was implanted in the control group. Cartilage-preserving group was implanted with chondrocyte-preserving laryngeal scaffold to compare the repair effect between the two groups. Results (1) after the two scaffolds were treated with acellular perfusion, the whole laryngeal structure was intact, and the volume of the whole larynx was not changed obviously. (2) the chondrogenic acellular laryngeal scaffold was not filled with surface cellular tissue, and collagen was found in the scaffold. The interstitial space composed of extracellular matrix components, with a tight cell structure, which exists on the surface of epiglottis, vocal cord, annular cartilage, etc. In the control group, the cellular components of the scaffold tissue were completely removed, but reticular connections were observed.
