心臟射血導(dǎo)致重力變化信號(hào)的人體實(shí)驗(yàn)研究

【摘要】:心臟疾病的預(yù)防、診斷、治療及預(yù)后的關(guān)鍵是準(zhǔn)確地獲取心臟功能信息。目前臨床常規(guī)的心臟功能檢測(cè)技術(shù)如心電圖、超聲心動(dòng)圖及冠脈造影等已經(jīng)成熟,廣泛應(yīng)用。但是這些檢測(cè)技術(shù)仍有很多的缺點(diǎn),如靈敏度不高、操作復(fù)雜、費(fèi)用高、特異性及有創(chuàng)等。本研究探索檢測(cè)心功能的新技術(shù),利用清華大學(xué)醫(yī)療新技術(shù)實(shí)驗(yàn)室基于心臟射血重力信號(hào)即心沖擊的心臟功能檢測(cè)技術(shù)而研發(fā)的重力心動(dòng)信號(hào)檢測(cè)系統(tǒng),對(duì)常見的心臟病病人進(jìn)行了初步的臨床醫(yī)學(xué)試驗(yàn),并對(duì)檢測(cè)結(jié)果進(jìn)行分析和處理。 本文首先介紹了心沖擊圖的產(chǎn)生原理及國(guó)內(nèi)外研究進(jìn)展,心沖擊信號(hào)的特征等。基于心沖擊信號(hào)的理論基礎(chǔ),介紹了清華大學(xué)醫(yī)療新技術(shù)實(shí)驗(yàn)室研發(fā)的重力心動(dòng)圖儀的硬件系統(tǒng)和軟件的采集分析功能。硬件部分包括重力信號(hào)傳感器、心電導(dǎo)聯(lián)、心音探頭、脈搏探頭和硬件電路。將心電、心音、和脈搏信號(hào)與心沖擊圖同步采集,以作為研究心臟病人BCG信號(hào)特征的時(shí)間及檢測(cè)時(shí)心臟信息的參考。為了能全面掌握心臟病人的信息,本文對(duì)心臟的生理功能和常見心臟病的生理病理特征做了簡(jiǎn)單介紹。 本文最主要的工作是利用重力心動(dòng)圖儀對(duì)常見心臟病人進(jìn)行了初步醫(yī)學(xué)實(shí)驗(yàn),并對(duì)檢測(cè)結(jié)果進(jìn)行了處理和分析。共80位心臟病人接受了重力心動(dòng)圖儀的檢測(cè),其中包括冠心病不穩(wěn)定性心絞痛病人30例,冠心病心肌缺血病人13例,急性冠脈綜合癥病人7例,急性心肌梗死病人1例,高血壓病人7例,高血壓心臟病2例,風(fēng)濕性心臟病2例,主動(dòng)脈瓣狹窄病人1例,心律失常病人8例,其他9例等。本文綜合病人的心電圖、動(dòng)態(tài)心電圖、超聲心動(dòng)圖等臨床檢查結(jié)果,利用與心沖擊信號(hào)同步采集的心電、心音、脈搏等信息做為心動(dòng)周期的參考,總結(jié)了常見心臟病心沖擊信號(hào)的特征,并對(duì)H/J值與左房增大的關(guān)系、I/K值與高血壓的關(guān)系進(jìn)行了研究和分析。結(jié)果表明,利用該重力信號(hào)檢測(cè)系統(tǒng)能穩(wěn)定可靠地獲取心沖擊信號(hào),并且能夠從心沖擊信號(hào)的分析中獲得某些常見心臟病的信號(hào)特征。
[Abstract]:The key to prevention, diagnosis, treatment and prognosis of heart disease is to obtain accurate cardiac function information. At present, clinical routine cardiac function testing techniques such as electrocardiogram, echocardiography and coronary angiography have been mature and widely used. However, these detection techniques still have many disadvantages, such as low sensitivity, complex operation, high cost, specificity and invasiveness. In this study, a new technique for detecting cardiac function was explored, and a gravity cardiac signal detection system based on cardiac ejection gravity signal was developed by Tsinghua University Medical New Technology Laboratory. A preliminary clinical medical trial was carried out on common heart disease patients, and the results were analyzed and treated. In this paper, we first introduce the principle of cardiogram, the research progress at home and abroad, and the characteristics of cardiac shock signal. Based on the theoretical basis of cardiac impact signal, this paper introduces the hardware system and software acquisition and analysis function of gravity cardiogram developed by Tsinghua University Medical New Technology Laboratory. The hardware part includes gravity signal sensor, ECG lead, heart sound probe, pulse probe and hardware circuit. Electrocardiogram (ECG), heart sound (ECG) and pulse signal were collected synchronously with percussive cardiogram (CPG), which could be used as a reference for studying the characteristics of BCG signal in cardiac patients and detecting cardiac information. In order to master the information of heart patients, this paper briefly introduces the physiological function of heart and the physiological and pathological characteristics of common heart diseases. The main work of this paper is to use the gravity cardiogram to carry on the preliminary medical experiment to the common heart patient, and has carried on the processing and the analysis to the test result. A total of 80 patients were examined by gravity cardiogram, including 30 patients with unstable angina pectoris, 13 patients with myocardial ischemia, 7 patients with acute coronary syndrome and 1 patient with acute myocardial infarction. There were 7 cases of hypertension, 2 cases of hypertensive heart disease, 2 cases of rheumatic heart disease, 1 case of aortic stenosis, 8 cases of arrhythmia and 9 cases of others. In this paper, the clinical results of ECG, ambulatory electrocardiogram, echocardiography and so on were synthesised. The information of ECG, heart sound, pulse and so on, which were collected synchronously with the pulse signal, was used as the reference for the cardiac cycle. The characteristics of heart shock signal in common heart disease were summarized, and the relationship between H / J value and left atrial enlargement was studied and analyzed. The results show that the gravity signal detection system can obtain the heart impact signal stably and reliably, and can obtain the signal characteristics of some common heart disease from the analysis of the heart impact signal.
