人羊膜治療家兔皮膚Ⅲ度燒傷的作用與抗炎機(jī)制研究

【摘要】:目的:探索人羊膜治療家兔皮膚Ⅲ度燒傷的作用和抗炎機(jī)制。 方法:熱銅棒燙傷致兔腹背部皮膚Ⅲ度燒傷。切痂后,左側(cè)創(chuàng)面分別帖敷新鮮人羊膜、涂抹rhEGF凝膠,右側(cè)創(chuàng)面僅涂生理鹽水。觀(guān)測(cè)燒傷創(chuàng)面愈合率和創(chuàng)面痊愈時(shí)間,并行創(chuàng)面組織病理學(xué)檢查。ELISA法檢測(cè)早期創(chuàng)面組織中IL-1、IL-6、IL-8、IL-10、IL-12和TNF-α含量。 結(jié)果:①治療第3d、8d和18d,羊膜組燒傷創(chuàng)面愈合率分別為(35.78±6.75)%、(71.21±6.63)%和(96.65±1.42)%,rhEGF凝膠組則為(23.75±3.57)%、(63.33±4.18)%和(92.79±1.80)%,對(duì)照組愈合率為(20.96±5.61)%、54.22±9.74)%和(90.73±2.15)%,羊膜組創(chuàng)面愈合率均顯著高于rhEGF組(P0.05)和對(duì)照組(P0.01);羊膜組、rhEGF組、對(duì)照組創(chuàng)面分別于18.40±1.26d、20.20±1.69d和22.13±1.25d痊愈,羊膜組的愈合時(shí)間較rhEGF組和對(duì)照組顯著縮短(P0.05)。②治療第3d,羊膜組創(chuàng)面組織紅腫較rhEGF組和對(duì)照組明顯減輕。病理學(xué)檢查顯示,羊膜組的組織水腫明顯較rhEGF組和對(duì)照組輕,炎細(xì)胞浸潤(rùn)也最少。③治療第3和8d,羊膜組創(chuàng)面組織中IL-1、IL-6、 IL-8、IL-12和TNF-a含量顯著低于rhEGF組和對(duì)照組(P0.05或P0.01),IL-10含量明顯高于rhEGF組和對(duì)照組(P0.05或P0.01)。 結(jié)論:人羊膜能促進(jìn)家兔皮膚Ⅲ度燒傷的愈合,在創(chuàng)面愈合早期通過(guò)下調(diào)創(chuàng)面組織中IL-1、IL-6、IL-8、IL-12和TNF-α的含量,上調(diào)IL-10的含量而發(fā)揮明顯抗炎作用,減輕創(chuàng)面炎癥反應(yīng)。
[Abstract]:Objective: to explore the effect and anti-inflammatory mechanism of human amniotic membrane in treating the third degree burn of rabbit skin.Methods: the third degree burn of rabbit's abdominal back skin was caused by scalding with hot copper rod.After escharectomy, the left wound was coated with fresh human amniotic membrane and rhEGF gel, while the right wound was only coated with normal saline.The wound healing rate and wound healing time were observed, and the contents of IL-10 IL-12 and TNF- 偽 in early wound tissues were detected by Elisa and histopathological examination.The wound healing time in the amniotic membrane group was significantly shorter than that in the rhEGF group and the control group on the 3rd day, and the redness of the wound tissue in the amniotic membrane group was significantly less than that in the rhEGF group and the control group.Histopathological examination showed that the tissue edema in amniotic membrane group was significantly lighter than that in rhEGF group and control group.On the 3rd and 8th day after the treatment, the contents of IL-1IL-6, IL-8 IL-12 and TNF-a in the amniotic membrane group were significantly lower than those in the rhEGF group and the control group (P0.05 or P0.01). The contents of IL-10 in the amniotic membrane group were significantly higher than those in the rhEGF group and the control group (P0.05 or P0.01).Conclusion: human amniotic membrane can promote the healing of third degree burn of rabbit skin. In the early stage of wound healing, it can reduce the inflammatory reaction of wound by down-regulating the contents of IL-6, IL-8, IL-12 and TNF- 偽 in the wound tissue, upregulating the content of IL-10.
