【摘要】:肝臟疾病是威脅人類生命健康的重大疾病之一。慢性肝病隨著病情如不能得到及時(shí)有效的控制將發(fā)展演變,產(chǎn)生肝臟細(xì)胞受損,,影響肝臟功能,甚至纖維化、癌變。當(dāng)病人患有肝硬化、肝內(nèi)膽管結(jié)石、肝癌、以及肝臟巨大血管瘤等疾病時(shí),實(shí)施肝臟切除、移植手術(shù)或使用高副作用藥物有可能導(dǎo)致患者肝功能衰竭甚至死亡。因此在采取這些措前應(yīng)準(zhǔn)確評(píng)估肝臟機(jī)能,對(duì)其風(fēng)險(xiǎn)有提前預(yù)估計(jì)可以有效增加治療成功率降低手術(shù)死亡率。 肝臟儲(chǔ)備功能是指被測(cè)者所有肝臟細(xì)胞最大功能的總和,是衡量人體肝臟細(xì)胞活性、健康度的重要指標(biāo),直接反映肝臟功能潛力,F(xiàn)在臨床上仍然廣泛使用吲哚氰綠排泄實(shí)驗(yàn)作為肝臟儲(chǔ)備功能分析方法。但傳統(tǒng)的人工采血標(biāo)定法由于其操作復(fù)雜性及效率低下等因素醫(yī)患雙方都帶來一定困難,所以其方法的改進(jìn)工作顯得尤為必要。利用脈搏色濃度法的儀器實(shí)現(xiàn)為肝臟儲(chǔ)備功能分析提供一種操作簡(jiǎn)便、創(chuàng)傷較小、連續(xù)、精確地測(cè)量方法。目前同類產(chǎn)品有日本光電公司生產(chǎn)的DDG3300K,其在世界范圍內(nèi)得到廣泛的應(yīng)用。 本課題從以下兩個(gè)方面做了一些研究工作,并取得一定進(jìn)展。 1.研究了脈搏色素濃度法,及其測(cè)量血液中吲哚氰綠數(shù)學(xué)物理模型及其修正方法,并以此為基礎(chǔ)進(jìn)一步討論分析肝臟儲(chǔ)備功能相關(guān)參數(shù)。 2.研發(fā)了基于脈搏色素濃度法的肝臟儲(chǔ)備功能分析儀器原理樣機(jī),研究了微弱脈搏信號(hào)采集、調(diào)理、調(diào)制解調(diào)等電子線路模塊,將測(cè)量原理的數(shù)學(xué)物理模型編寫成計(jì)算機(jī)能夠處理運(yùn)行的軟件代碼。基本實(shí)現(xiàn)了肝臟儲(chǔ)備功能分析儀器功能,儀器具備進(jìn)一步校準(zhǔn)、增加精度、擴(kuò)充功能乃至實(shí)用化商品化的發(fā)展?jié)撡|(zhì)。 本文內(nèi)容提要如下: 第一章從課題研究?jī)?nèi)容及背景知識(shí)方面介紹了肝臟儲(chǔ)備功能評(píng)估在肝病診斷治療尤其手術(shù)風(fēng)險(xiǎn)評(píng)估中的重要意義。同時(shí)歸納總結(jié)了目前國(guó)內(nèi)外對(duì)肝臟儲(chǔ)備功能評(píng)估方法及測(cè)量?jī)x器設(shè)備的研究。 第二章介紹脈搏色素濃度法的數(shù)學(xué)物理模型及補(bǔ)償方法。這是儀器原理模型,是儀器研發(fā)的方法基礎(chǔ),也是未來儀器改進(jìn)升級(jí)的理論依據(jù)。 第三章介紹了儀器硬件設(shè)計(jì)。包括光學(xué)傳感器、各級(jí)模擬調(diào)理電路和數(shù)據(jù)采集系統(tǒng)。并且從信號(hào)時(shí)頻兩域?qū)Ω麟A段信號(hào)給出系統(tǒng)性的分析。 第四章介紹了吲哚氰綠濃度曲線計(jì)算、15分鐘滯留率和血漿消失率等重要肝儲(chǔ)備分析參數(shù)的計(jì)算及軟件實(shí)現(xiàn)。 第五章主要是數(shù)據(jù)的出路與分析,把本儀器測(cè)量數(shù)據(jù)與DDG-3300k測(cè)得數(shù)據(jù)對(duì)比,尋找相關(guān)性。結(jié)果與測(cè)得數(shù)據(jù)顯著相關(guān),相關(guān)系數(shù)超過0.96。 第六章對(duì)全文總結(jié)概括,并給出了下一步改進(jìn)意見。
[Abstract]:Liver disease is one of the major diseases threatening human life and health. Chronic liver disease will develop and evolve with the condition not being controlled effectively and timely, resulting in liver cell damage, affecting liver function, and even fibrosis. Cancer. When patients suffer from liver cirrhosis, intrahepatic cholelithiasis, liver cancer, and giant hemangioma of the liver, liver resection is performed. Transplant surgery or the use of high side effects drugs can lead to liver failure or even death in patients. Therefore, liver function should be accurately assessed before taking these measures. Early estimation of the risk can effectively increase the success rate of treatment and reduce operative mortality. Liver reserve function refers to the sum of the maximum function of all liver cells tested, and it is an important index to measure the activity and health of human liver cells. Indocyanine green excretion test is still widely used as the method of liver reserve function analysis. But the traditional artificial blood sampling calibration method is due to its operational complexity and inefficiency and other factors. Both doctors and patients have brought some difficulties. Therefore, it is necessary to improve the method. The instrument of pulse color concentration method is used to provide a simple operation, less trauma and continuity for the analysis of liver reserve function. At present, the similar products are DDG 3300K produced by Japan Optoelectronic Company, which is widely used in the world. This subject has done some research work from the following two aspects, and has made certain progress. 1. The pulse pigment concentration method, the mathematical and physical model for measuring indocyanine green in blood and its correction method were studied, and the parameters related to liver reserve function were discussed and analyzed. 2. The principle prototype of liver reserve function analysis instrument based on pulse pigment concentration method is developed, and the electronic circuit modules such as weak pulse signal acquisition, conditioning, modulation and demodulation are studied. The mathematical and physical model of the measurement principle is compiled into the software code which the computer can process and run. The function of the liver reserve function analysis instrument is basically realized. The instrument has the ability to further calibrate and increase the precision. Expansion of functions and even practical commercialization of the development potential. The summary of this article is as follows:. The first chapter introduces the significance of liver reserve function evaluation in the diagnosis and treatment of liver diseases, especially in the risk assessment of liver surgery from the aspects of research contents and background knowledge. At the same time, it summarizes the current liver reserve function at home and abroad. Research on evaluation methods and measuring instruments and equipment. The second chapter introduces the mathematical and physical model and compensation method of pulse pigment concentration method, which is the principle model of instrument, the method foundation of instrument research and development, and the theoretical basis for the improvement and upgrading of instrument in the future. The third chapter introduces the hardware design of the instrument, including optical sensor, analog conditioning circuit and data acquisition system. In chapter 4th, the calculation and software realization of important hepatic reserve parameters such as 15 minute retention rate and plasma disappearance rate of indocyanine green concentration curve were introduced. Chapter 5th is mainly about the way out and analysis of the data. Comparing the data measured by this instrument with the data measured by DDG-3300k to find the correlation, the result is significantly related to the measured data, and the correlation coefficient is more than 0.96. Chapter 6th summarizes the full text and gives suggestions for further improvement.
