【摘要】: 隨著心臟類疾病患者數(shù)量的逐步增加,在現(xiàn)代社會(huì)中,心臟類疾病已經(jīng)日益成為人類健康的最主要威脅,而對(duì)于其中最嚴(yán)重的末期充血性心力衰竭,傳統(tǒng)器官移植手術(shù)治療手段由于心臟器官供體的極度缺乏,已經(jīng)遠(yuǎn)不能滿足患者的需求。因此,全人工心臟特別是具備全植入特征的第二代電動(dòng)全人工心臟技術(shù)的研究,便受到了各國(guó)研究人員的廣泛關(guān)注,比如美國(guó)國(guó)家衛(wèi)生協(xié)會(huì)(NIH)自1990年代開始就對(duì)該系統(tǒng)的研究給予了專門的支持,并且在2004年開始有公司進(jìn)入臨床實(shí)驗(yàn)階段的報(bào)告。2006年8月FDA首次批準(zhǔn)每年允許4000個(gè)美國(guó)馬薩諸塞州丹弗市的Abiomed公司生產(chǎn)的“AbioCor”人工心臟用于心臟移植。而在國(guó)內(nèi),除了一些零散的機(jī)械循環(huán)支持系統(tǒng)的研究外,目前尚未見到系統(tǒng)的開展全人工心臟研究的報(bào)導(dǎo)。而全人工心臟測(cè)試系統(tǒng)是與全人工心臟的研制相配套的,在其進(jìn)行動(dòng)物試驗(yàn)之前,對(duì)全人工心臟原型樣機(jī)整體性能參數(shù)進(jìn)行測(cè)試的必要系統(tǒng)。因此,國(guó)外關(guān)于全人工心臟測(cè)試系統(tǒng)的報(bào)告很少,而國(guó)內(nèi)關(guān)于全人工心臟測(cè)試系統(tǒng)的研究還未見報(bào)道。 本文報(bào)告了一種配套用于往復(fù)式推板結(jié)構(gòu)的電動(dòng)全人工心臟測(cè)試的系統(tǒng)的設(shè)計(jì)與實(shí)現(xiàn),并且提出了該測(cè)試系統(tǒng)的數(shù)學(xué)模型、建模方法、制作方案及數(shù)據(jù)采集系統(tǒng)的硬件搭建、數(shù)據(jù)采集程序的研制。該系統(tǒng)與國(guó)外現(xiàn)有研究系統(tǒng)屬于同一個(gè)技術(shù)代,類似系統(tǒng)在國(guó)內(nèi)尚未見報(bào)道。 其主要特點(diǎn)有:1)提出了一種新的基于彈性腔模型的人體血液循環(huán)系統(tǒng)整體特性集中參數(shù)模型。把原二元件模型中的電阻、電容替換為對(duì)應(yīng)參數(shù)的彈性腔、阻尼閥,采用類似于二元件模型的建模方法進(jìn)行數(shù)學(xué)建模。該模型有以下一些特點(diǎn):將血液循環(huán)系統(tǒng)作為一個(gè)閉合體系,強(qiáng)調(diào)其內(nèi)部各器官間的相互作用以及循環(huán)系統(tǒng)與外部作用的整體性反應(yīng),在各組成部分的特性描述上則進(jìn)行較多的簡(jiǎn)化;參數(shù)少、生理意義明確、計(jì)算容易且便于制作物理模型;比起一般二元件模型或Frank集中參數(shù)模型所采用的電阻、電容器件模擬辦法,該模型直接對(duì)血液流路器件進(jìn)行模擬,更能逼近測(cè)試系統(tǒng)實(shí)物的真實(shí)情況。2)測(cè)試系統(tǒng)采用有機(jī)玻璃制作,為分體式結(jié)構(gòu),便于實(shí)驗(yàn)觀測(cè)也方便各種傳感器及輔助裝置的安裝、維護(hù)及升級(jí),為整個(gè)系統(tǒng)得以小型化、標(biāo)準(zhǔn)化打下了基礎(chǔ)。3)測(cè)試系統(tǒng)的參數(shù)可調(diào)范圍寬,參數(shù)調(diào)節(jié)方式多樣,不僅可以模擬出人體血液循環(huán)系統(tǒng)正常時(shí)候的狀態(tài),還可模擬出人體高血壓、低血壓的狀態(tài),加大了本系統(tǒng)的應(yīng)用范圍。4)測(cè)試程序與控制程序相結(jié)合,除實(shí)現(xiàn)了壓力和流路流量的實(shí)時(shí)檢測(cè)、記錄外,還可對(duì)推板水平位移進(jìn)行實(shí)時(shí)監(jiān)控,增強(qiáng)了對(duì)全人工心臟檢測(cè)與監(jiān)控的直觀性、方便性。 多次使用本系統(tǒng)對(duì)配套的全人工心臟進(jìn)行的測(cè)試實(shí)驗(yàn)表明,本系統(tǒng)操作使用方便、測(cè)試數(shù)據(jù)準(zhǔn)確可信,可重復(fù)性好。為國(guó)內(nèi)外全人工心臟及其相關(guān)領(lǐng)域的研究提供了新的參考方案。 本文的研究?jī)?nèi)容得到了重慶市科委科技攻關(guān)項(xiàng)目(csts.201ac3031)以及重慶大學(xué)學(xué)科建設(shè)項(xiàng)目的資助。
[Abstract]:With the number of heart disease patients increased gradually in modern society, heart disease has become the main threat to human health, and for end-stage congestive heart failure is one of the most serious, the traditional therapy of organ transplants because the heart of the shortage of donor organs has been unable to meet the needs of the patients. Therefore, all especially the research of artificial heart implantation with full features of the second generation of electric total artificial heart technology, it is widely concerned by researchers all over the world, such as the National Institutes of Health (NIH) since 1990s began the study of the system has given special support, and in 2004 the company began to have entered clinical trials report.2006 FDA for the first time in August approved the annual permits 4000 Massachusetts Denver City Abiomed company production of "AbioCor" artificial heart to heart Dirty transplantation. This paper reports the design and implementation of a reciprocating push plate structure of the electric total artificial heart test system for supporting, and puts forward the mathematical model of the test system modeling method, production program and data acquisition system's hardware, developed a data acquisition program. This system and the existing foreign research system belongs to the same generation technology, similar system has not been reported in China.
The main features are: 1) proposed a new lumped parameter model of the overall characteristics of the human circulatory system based on elastic cavity model. Resistance to the original two components in the model, the elastic cavity is replaced with the corresponding parameters, capacitance damping valve, the modeling method is similar to the two element model for mathematical modeling of the model. Many experiments on the whole artificial heart carried out by the system show that the system is easy to operate and use, accurate and reliable, and reproducible. It provides a new reference for the research of the whole artificial heart and related fields at home and abroad.
