尋常型銀屑病患者血清IL-23和IL-27水平的表達

【摘要】:目的:銀屑病是一種以上皮細胞增生、分化失控和大量T細胞浸潤為特征的慢性復(fù)發(fā)性炎癥性皮膚病。銀屑病的發(fā)病機理目前尚不十分清楚,受遺傳、代謝異常及神經(jīng)心理等諸多因素的影響,免疫功能異常在其發(fā)病中具有重要作用。研究顯示輔助性T細胞亞群及其細胞因子在自身免疫病的發(fā)病及治療中均具有重要的作用。白介素(interleukin,IL)為一大類主要由活化的淋巴細胞及樹突狀細胞產(chǎn)生的細胞因子,具有多種生物學(xué)作用。大量研究表明IL-12家族各成員以及同其他細胞因子之間存在著相互協(xié)同、相互拮抗的網(wǎng)絡(luò),不僅在細胞內(nèi)感染及炎癥過程中起著重要的調(diào)節(jié)作用,而且與銀屑病、多發(fā)性硬化癥、Crohn,s病等多種臨床疾病的發(fā)病密切相關(guān)。目前為止,IL-12家族共有4個成員:IL-12、IL-23、IL-27和IL-35。其中IL-12、IL-23和IL-27主要由樹突狀細胞、單核-巨噬細胞和EBV轉(zhuǎn)化的B淋巴母細胞產(chǎn)生,IL-35主要由Treg產(chǎn)生。此外,也可由EBV轉(zhuǎn)化的B細胞和胎盤產(chǎn)生。 IL-23由p19和IL-12中的p40兩個亞基組成,它們之間以二硫鍵相結(jié)合。主要由活化的單核細胞、巨噬細胞(Mφ)和樹突狀細胞(DC)產(chǎn)生。近年有關(guān)IL-23與銀屑病皮損的相關(guān)性的研究較多,且主要研究方向為銀屑病皮損處IL-23表達水平與銀屑病嚴重程度的相關(guān)性研究以及IL-23與銀屑病皮損動物模型建立的相關(guān)研究。有關(guān)銀屑病患者血清中IL-23水平與疾病的相關(guān)性研究的報道較少。IL-27由EBI3和P28兩個亞基組成,其中EBI3與IL-12的p40亞基同源, p28與IL-12的p35亞基同源。IL-27主要產(chǎn)生于抗原呈遞細胞(單核細胞、巨噬細胞、樹突狀細胞),在單核細胞衍生的樹突狀細胞、活化的單核細胞和活化的巨噬細胞中水平最高。國內(nèi)尚未見有關(guān)銀屑病患者血清中IL-27水平與疾病嚴重程度的相關(guān)性研究報道。 本實驗通過檢測尋常型銀屑病患者外周血血清中的IL-23和IL-27水平與正常對照組比較,研究血清中IL-23、IL-27表達水平與尋常型銀屑病發(fā)病的相關(guān)性及與病情發(fā)展的關(guān)系,進一步探討銀屑病的發(fā)病機制,為銀屑病的預(yù)防和治療提供新思路。 方法:病例組:尋常型銀屑病患者29例,均來自2010年6月至2010年12月河北醫(yī)科大學(xué)第二醫(yī)院皮膚科門診,符合銀屑病的診斷標(biāo)準(zhǔn)。排除紅斑狼瘡、天皰瘡、皮肌炎等自身免疫性疾病以及其它嚴重系統(tǒng)性疾病,且三個月內(nèi)未應(yīng)用過免疫抑制劑或皮質(zhì)類固醇激素等影響自身免疫功能的藥物。并根據(jù)病情將病例組分為兩組:靜止期組8例,進展期組21例。對照組:11例健康者,來自河北醫(yī)科大學(xué)第二醫(yī)院健康工作人員,均無免疫性疾病及家族史,經(jīng)t檢驗,性別、年齡與病例組相比無顯著差異。由同一工作人員對病例組患者進行PASI評分,采集對照組和病例組靜脈血各約2ml,分離血清,采用ELISA法檢測血清中IL-23和IL-27表達水平的值,所得數(shù)據(jù)用SPSS17.0軟件進行統(tǒng)計學(xué)處理,因測得的IL-23的數(shù)據(jù)滿足正態(tài)性和方差齊性,故用完全隨機設(shè)計的兩均數(shù)比較的統(tǒng)計學(xué)方法;銀屑病患者組中的進展期組和靜止期IL-23表達水平滿足正態(tài)性和方差齊性,故也可采用上述方法進行統(tǒng)計學(xué)處理。病例組和正常對照組所測得的IL-27的數(shù)據(jù)均不具有正態(tài)性,用兩個獨立樣本的非參數(shù)檢驗分析兩組間差別。檢驗水準(zhǔn)均取P0.05有統(tǒng)計學(xué)意義。如病例組和正常對照組所得結(jié)果具有顯著性差異,采用兩變量的Pearson相關(guān)分析對病例組IL-23和IL-27表達水平與相應(yīng)的PASI評分水平進行相關(guān)分析,以P0.05作為有統(tǒng)計學(xué)意義。 結(jié)果: 1血清中IL-23濃度測定結(jié)果(單位pg/ml):尋常型銀屑病組平均值為(51.556±7.081),明顯高于正常對照組(17.862±6.639) (t=13.657, P0.05),差異有統(tǒng)計學(xué)意義,病例組中進展期組與靜止期組無顯著性差異(t=0.627,P0.05)。病例組IL-23與臨床PASI評分經(jīng)過直線相關(guān)性分析有正相關(guān)關(guān)系(r=0.629,P0.05)。 2血清中IL-27濃度測定結(jié)果(單位pg/ml)):病例組與對照組的數(shù)值分布均不滿足正態(tài)性,應(yīng)用非參數(shù)檢驗,兩組無統(tǒng)計學(xué)差別。U值為128.000, P=0.336,P0.05。 結(jié)論: 1尋常型銀屑病患者血清中IL-23表達水平高于正常對照組,表明IL-23的高表達為銀屑病患者免疫異常表現(xiàn)之一,證實了尋常型銀屑病的發(fā)病與T細胞免疫失衡有關(guān)。 2尋常型銀屑病血清中IL-23的表達水平在進展期組與靜止期組無顯著差異,說明IL-23的表達水平與銀屑病的進展無明確關(guān)系,不能作為反應(yīng)疾病進展情況的指標(biāo)。 3尋常型銀屑病血清中IL-23的表達水平與相應(yīng)的PASI評分的值存在直線相關(guān)關(guān)系,且為正相關(guān),說明血清中IL-23的表達水平可以反映疾病嚴重程度。 4尋常型銀屑病患者血清中IL-27表達水平與正常對照組差異無統(tǒng)計學(xué)意義,說明IL-27的表達水平與尋常型銀屑病的發(fā)病機制可能無明確關(guān)聯(lián),有待進一步研究。
