【摘要】：目的探討中老年人大皰性類天皰瘡(BP)合并臨床疾病的特點。方法收集2011年3月至2013年9月在該院門診就診及住院的BP及濕疹患者的臨床資料,對50例BP和49例濕疹患者的臨床資料進行比較分析。結果 BP平均發(fā)病年齡為73.5歲,診斷前病程平均3.4個月;老年濕疹發(fā)病平均年齡70.3歲,診斷前病程平均2.7個月,老年BP合并內科疾病包括神經系統疾病19例(38%),其中有腦血管病17例(腦血栓、腦出血、腦梗死等)和帕金森病1例、癡呆1例;高血壓7例(14%);糖尿病(DM)4例(8%);冠心病3例(6%);其他3例(6%)。老年濕疹伴發(fā)疾病包括:高血壓20例(40.8%);DM 8例(16.3%);冠心病7例(14%);腦血管疾病5例(10.2%);其他7例(14%)。結論老年人BP合并神經系統疾病的情況較多見。二者合并出現的機制尚不清楚,免疫交叉反應或與年齡相關的某些變性退化的過程可能激發(fā)BP的發(fā)病。老年人濕疹則合并高血壓、DM較多見,可能與體內代謝及內分泌變化及長期用降壓藥物及降糖藥物有關。
[Abstract]:Objective to investigate the clinical features of pemphigoid (BP) in middle-aged and elderly people. Methods from March 2011 to September 2013, the clinical data of 50 cases of BP and 49 cases of eczema were analyzed. The clinical data of BP and eczema patients who were treated and hospitalized in the hospital from March 2011 to September 2013 were collected. Results the mean age of onset of BP was 73. 5 years, and the mean course of disease was 3. 4 months before diagnosis. The average age of onset of eczema was 70.3 years old and the course of disease was 2.7months before diagnosis. 19 cases (38%) of senile BP complicated with internal diseases included nervous system diseases, among them 17 cases had cerebrovascular disease (cerebral thrombosis, cerebral hemorrhage). Cerebral infarction, etc.) and Parkinson's disease (1 case), dementia (1 case); There were 7 cases of hypertension (14%), 4 cases of diabetic (DM) (8%), 3 cases of coronary heart disease (6%) and 3 cases of others (6%). The concomitant diseases of eczema in the elderly included hypertension in 20 cases (40.8%); DM in 8 cases (16.3%), coronary heart disease in 7 cases (14%), cerebrovascular diseases in 5 cases (10.2%) and others in 7 cases (14%). Conclusion BP complicated with nervous system diseases is more common in the elderly. The mechanism of the combination is not clear, and the immune cross-reaction or some age-related degeneration processes may stimulate the pathogenesis of BP. Eczema is associated with hypertension in the elderly, and DM is more common, which may be related to metabolic and endocrine changes in the body and long-term use of antihypertensive drugs and hypoglycemic drugs.
