【摘要】：目的:本課題旨在檢測嗜酸性粒細(xì)胞增多性皮炎(hypereosinophilic dermatitis,HED)患者外周血中免疫球蛋白E(immunoglobulin E,Ig E)的水平和嗜酸性粒細(xì)胞(eosinophil cell,EOS)計(jì)數(shù)的相關(guān)性,為HED的臨床診斷及治療方案的制定提供實(shí)驗(yàn)數(shù)據(jù)支持。方法:本課題嚴(yán)格按照納入標(biāo)準(zhǔn)和排除標(biāo)準(zhǔn),挑選了2014年1月-2016年1月間就診于山西醫(yī)科大學(xué)第一臨床醫(yī)學(xué)院皮膚性病科進(jìn)行住院治療的HED患者(組成病例組)30例及在同時(shí)期于山西醫(yī)科大學(xué)第一臨床醫(yī)學(xué)院常規(guī)進(jìn)行體檢的體檢者(組成對照組)30例,應(yīng)用血球計(jì)數(shù)儀測定外周血中嗜酸性粒細(xì)胞的計(jì)數(shù)水平,應(yīng)用酶聯(lián)免疫吸附測定法(ELISA)測定其外周血中Ig E的水平。結(jié)果以EOS計(jì)數(shù)水平以大于1.5×109/L為異常檢出,計(jì)做(+),以小于1.5×109/L為陰性檢出,計(jì)做(-);Ig E水平其結(jié)果以大于120KU/L為異常檢出,計(jì)做(+),以小于120KU/L為陰性檢出,計(jì)做(-)。應(yīng)用SPSS22.0統(tǒng)計(jì)軟件進(jìn)行分析:計(jì)量資料采用均數(shù)數(shù)據(jù)±標(biāo)準(zhǔn)差(X±S)表示,兩組間計(jì)量資料的比較采用t檢驗(yàn)進(jìn)行分析,數(shù)據(jù)間相關(guān)性采用相關(guān)性分析,以P0.05表示差異具有統(tǒng)計(jì)學(xué)意義。結(jié)果:(1)病例組外周血中EOS計(jì)數(shù)水平和Ig E水平的異常檢出率依次為57%、93%;對照組外周血中Ig E水平的異常檢出率為3%。病例組的EOS計(jì)數(shù)高于對照組EOS計(jì)數(shù),其差異具有顯著性(t=3.502,P0.01)。病例組的Ig E計(jì)數(shù)高于對照組,其差異具有顯著性(t=7.922,P0.01)。即病例組外周血中EOS計(jì)數(shù)和Ig E水平均高于對照組。(2)通過統(tǒng)計(jì)學(xué)方法分析得出病例組及對照組外周血中EOS計(jì)數(shù)水平與Ig E水平經(jīng)過相關(guān)性分析呈相關(guān)性。病例組通過Spearman秩相關(guān)分析求得rs值為0.603,對照組通過Pearson相關(guān)性分析求得r值為0.697,并且兩組均為中度正相關(guān)關(guān)系,二者在檢測水平上的差別具有統(tǒng)計(jì)學(xué)意義(P0.05)。結(jié)論:檢測得Ig E在HED患者的體內(nèi)均呈高水平狀態(tài),且在HED患者中Ig E水平與EOS計(jì)數(shù)水平是呈中度正相關(guān)性。測定其值有助于HED的臨床觀察與治療。
[Abstract]:Objective: to detect the correlation between the level of immunoglobulin (E (immunoglobulin E, IgE) and eosinophil (eosinophil cell,EOS) count in peripheral blood of patients with eosinophilic dermatitis (hypereosinophilic dermatitis,HED). To provide experimental data support for the clinical diagnosis and treatment plan of HED. Methods: the subject is strictly in accordance with the inclusion criteria and exclusion criteria. From January 2014 to January 2016, 30 HED patients who were hospitalized in the Department of Dermatology, first Clinical Medical College of Shanxi Medical University and the first clinical doctor of Shanxi Medical University during the same period were selected. There were 30 physical examiners (control group) who underwent routine physical examination in the college. The levels of eosinophils in peripheral blood and Ig E in peripheral blood were measured by blood cell counter and enzyme linked immunosorbent assay (ELISA) respectively. Results the abnormal detection level of EOS was more than 1.5 脳 109 渭 L, the negative detection of (), was less than 1.5 脳 109 渭 L, and the count was (-). The results of Ig E level were greater than 120KU/L as abnormal detection, (), was less than 120KU/L as negative detection, and (-) was counted as (-). SPSS22.0 statistical software was used to analyze: the measurement data was expressed by mean data 鹵standard deviation (XS), the comparison of measurement data between the two groups was analyzed by t-test, and the correlation between data was analyzed by correlation analysis. The difference was statistically significant (P0.05). Results: (1) the abnormal detection rates of EOS count and Ig E in peripheral blood were 57% and 93% in the case group and 3% in the control group, respectively. The EOS count in the case group was higher than that in the control group, and the difference was significant (t = 3.502, P0.01). The Ig E count in the case group was significantly higher than that in the control group (t = 7.922, P0.01). That is to say, the EOS count and Ig E level in the peripheral blood of the case group were higher than those of the control group. (2) the correlation between the EOS count level and the Ig E level in the peripheral blood of the patient group and the control group was analyzed by statistical analysis. The rs of the case group was 0.603 by Spearman rank correlation analysis, and that of the control group was 0.697 by Pearson correlation analysis, and both of the two groups were moderate positive correlation. The difference in detection level between the two was statistically significant (P0.05). Conclusion: the level of Ig E is high in patients with HED, and there is a moderate positive correlation between the level of Ig E and the level of EOS in patients with HED. The determination of its value is helpful to the clinical observation and treatment of HED.
