【摘要】：目的：(1)檢測β-防御素2 (HBD-2)、TNF-α、NF-кB p65在尋常型銀屑病皮損中的表達,研究HBD-2、TNF-α、NF-кB p65的表達與銀屑病嚴重程度的關(guān)系；(2)探討三者在銀屑病發(fā)生發(fā)展中的作用。 方法：(1)采用免疫組化SP法(鏈霉菌抗生物素蛋白-過氧化物酶連接法)檢測45例銀屑病患者皮損組織、45例銀屑病患者非皮損組織及15例正常皮膚組織標(biāo)本中HBD-2、TNF-α、NF-кB p65的表達,比較不同組別間三者表達的差異,并對HBD-2、TNF-α、NF-кB p65進行相關(guān)性分析。(2)根據(jù)PASI評分將45例銀屑病患者分為輕、中、重三組(輕度25例、中度12例、重度8例),檢測輕、中、重銀屑病患者HBD-2、TNF-α、NF-кB p65的表達,比較不同組別間三者表達的差異,分析三者在銀屑病皮損中的相關(guān)性。 結(jié)果：(1)與正常人皮膚組織和銀屑病患者非皮損區(qū)相比,銀屑病患者皮損區(qū)HBD-2、TNF-α、NF-кB p65的表達顯著增強,非皮損區(qū)HBD-2、TNF-α的表達也高于正常人皮膚組織,差異均有統(tǒng)計學(xué)意義(P0.05),非皮損區(qū)NF-кB p65的表達與正常人皮膚組織相比差異無統(tǒng)計學(xué)意義(P0.05)。在正常皮膚組織、尋常型銀屑病非皮損區(qū)和銀屑病皮損區(qū)之間HBD-2與TNF-α, TNF-與NF-кB p65, HBD-2與NF-кB p65均存在正相關(guān)(P0.05)。(2)在銀屑病皮損區(qū),對HBD-2、TNF-α, NF-кB p65在輕度組、中度組、重度組的表達進行比較,重度組銀屑病患者三者的表達高于中度組,中度組又高于輕度組,并具有統(tǒng)計學(xué)意義(P0.05)。 結(jié)論：1、HBD-2, TNF-α在正常皮膚組織、尋常型銀屑病非皮損區(qū)、尋常型銀屑病皮損區(qū)中的表達逐漸增強,NF-кB p65在尋常型銀屑病皮損區(qū)顯著高表達。2、HBD-2, TNF-α, NF-кB p65在尋常型銀屑病皮損中的表達均升高,且皮損愈重,表達愈強；HBD-2與TNF-α, TNF-α與NF-кB p65, HBD-2與NF-кB p65表達水平之間均存在正相關(guān)。 綜上所述,HBD-2, TNF-α, NF-кB p65與銀屑病的發(fā)生、發(fā)展密切相關(guān),且三者呈協(xié)同作用。三者相互促進、相互增強,加重了銀屑病皮損。
[Abstract]:Objective: (1) to detect the expression of 尾 -defensin 2 (HBD-2), TNF- 偽 and NF- B p65 in psoriatic lesions, and to study the relationship between the expression of HBD-2,TNF- 偽, NF- B p65 and the severity of psoriasis. (2) to explore the role of three factors in the occurrence and development of psoriasis. Methods: (1) Immunohistochemical SP method was used to detect HBD-2, in 45 cases of psoriasis, 45 cases of psoriasis and 15 cases of normal skin tissue. The expression of TNF- 偽 and NF- B p65 were compared among different groups, and the correlation between HBD-2,TNF- 偽 and NF- B p65 was analyzed. (2) according to PASI score, 45 patients with psoriasis were divided into mild and moderate psoriatic patients. The expression of HBD-2,TNF- 偽 and NF- B p65 in patients with mild, moderate and severe psoriasis were detected in three groups (mild 25 cases, moderate 12 cases, severe 8 cases). To analyze the correlation among the three factors in psoriatic lesions. Results: (1) the expression of HBD-2,TNF- 偽 and NF- B p65 in psoriatic lesions was significantly higher than that in normal skin tissues and psoriatic lesions, and the expression of HBD-2, in non-lesion areas was significantly higher than that in normal skin tissues and psoriatic lesions. The expression of TNF- 偽 was significantly higher than that of normal skin tissue (P0.05), while the expression of NF- B p65 in non-lesion area was not significantly different from that in normal skin tissue (P0.05). In normal skin tissue, there was a positive correlation between HBD-2 and TNF- 偽, TNF- and NF- B p65, HBD-2 and NF- B p65 between psoriasis vulgaris and psoriasis lesions (P0.05). (2). The expression of HBD-2,TNF- 偽 and NF- B p65 in mild group, moderate group and severe group were compared. The expression of HBD-2,TNF- 偽 in severe group was higher than that in moderate group, and the expression in moderate group was higher than that in mild group (P0.05). Conclusion: 1 the expression of HBD-2 and TNF- 偽 in normal skin, psoriasis vulgaris, psoriasis vulgaris and psoriasis vulgaris were gradually increased, and the expression of NF- p65 in psoriasis vulgaris was significantly higher than that in psoriasis vulgaris, and the expression of HBD-2in psoriasis vulgaris was significantly higher than that in psoriasis vulgaris. The expressions of TNF- 偽 and NF- p65 in psoriasis vulgaris were all increased, and the more severe the lesions, the stronger the expression. There was a positive correlation between the expression levels of HBD-2 and TNF- 偽, TNF- 偽 and NF- B p65, HBD-2 and NF- B p65. In conclusion, HBD-2, TNF- 偽, NF- B p65 are closely related to the occurrence and development of psoriasis, and they have synergistic effects. The three promote each other, reinforce each other and aggravate psoriatic lesions.
