【摘要】：報(bào)告3例大皰性類天皰瘡(BP)180抗體陽性的尋常型天皰瘡患者。所有患者均以口腔、外陰黏膜糜爛,軀干多發(fā)水皰逐漸加重就診。皮膚科檢查:患者分別在頭皮、面部、頸部、口腔、外陰、軀干及下肢等部位出現(xiàn)綠豆至花生米大的孤立或融合性水皰,部分糜爛面潮濕。實(shí)驗(yàn)室檢查:血清檢測抗橋粒芯糖蛋白(Dsg)抗體和Bp180抗體均陽性,直接免疫熒光(DIF)示抗棘細(xì)胞橋�？贵w(PCDA)陽性或抗表皮基膜抗體(EBA)陽性。皮損組織病理檢查:表皮內(nèi)水皰,棘細(xì)胞松解,基底呈"絨毛"樣改變,基底層見中量淋巴細(xì)胞浸潤。結(jié)合臨床及實(shí)驗(yàn)室檢查診斷為BP180抗體陽性的尋常型天皰瘡。
[Abstract]:Three cases of pemphigoid vulgaris with positive (BP) 180 antibody were reported. All patients with oral cavity, vulvar mucosal erosion, multiple blister trunk gradually aggravated. Dermatology examination: patients in the scalp, face, neck, oral cavity, vulva, trunk and lower limbs and other parts of mung bean to peanut large isolated or fused blister, part of the erosive surface moist. Laboratory tests showed that the anti-desmosome glycoprotein (Dsg) antibody and Bp180 antibody were positive in serum, and direct immunofluorescence (DIF) showed (PCDA) positive for anti-desmosome antibody or (EBA) positive for anti-epidermal basement membrane antibody. Histopathological examination of the lesions: blister in epidermis, loosening of spinous cells, "villi" like changes in basement, and infiltration of lymphocytes in the basal layer. Combined with clinical and laboratory examination, BP180 antibody positive pemphigus vulgaris was diagnosed.
