單純皰疹病毒2型感染與缺血性腦卒中不良預后的關系

【摘要】：目的探討單純皰疹病毒2型(herpes simplex virus 2,HSV-2)感染與缺血性腦卒中發(fā)病后14 d(或出院)時不良預后的關系。方法 2011年6月~2013年12月,本研究采用隊列研究的方法,從國內14家醫(yī)院納入確診的缺血性腦卒中病例1 150例,檢測入院時血清HSV-2 Ig G水平,以發(fā)病后14 d(或出院)發(fā)生死亡或心血管事件為不良預后,采用Logistic回歸方法分析HSV-2 Ig G水平與不良預后的關系。結果依HSV-2 Ig G四分位數將病例分為4組,以第1分位組為參照組,經多因素調整后第4分位組發(fā)生不良預后的風險是參照組的4.32倍(95%CI:1.33~14.03,P=0.015),且隨不同組間HSV-2 Ig G水平的增加,不良預后的發(fā)生風險有增加的趨勢。亞組分析發(fā)現,病例為女性、或不飲酒、或伴有糖尿病、或伴有高血壓時,高水平HSV-2 Ig G可增加14 d(或出院)時不良預后的發(fā)生風險(均有P0.05)。結論缺血性腦卒中伴HSV-2感染時,高水平的HSV-2 Ig G能增加病后14 d(或出院)時不良預后的發(fā)生風險,且隨HSV-2 Ig G水平的增加,不良預后的發(fā)生風險有增加的趨勢。
[Abstract]:Objective to investigate the relationship between herpes simplex virus 2 (HSV-2) infection and poor prognosis at 14 days after ischemic stroke. Methods from June 2011 to December 2013, a cohort study was conducted in 1 150 patients with ischemic stroke from 14 hospitals in China. Serum HSV-2 Ig G levels were measured at admission. Taking death or cardiovascular events 14 days after onset or discharge as the adverse prognosis, the relationship between HSV-2 Ig G level and adverse prognosis was analyzed by Logistic regression method. Results the patients were divided into 4 groups according to the HSV-2 Ig G quartile. The risk of adverse prognosis in the 4th quartile group after multivariate adjustment was 4.32 times higher than that in the control group (95 CI: 1.3314.03 P 0.015). The risk of adverse prognosis increased with the increase of HSV-2 Ig G level in different groups. Subgroup analysis showed that high level of HSV-2 Ig G increased the risk of adverse prognosis at 14 d (or discharge) in female patients who did not drink alcohol or had diabetes mellitus or hypertension (P0.05). Conclusion in ischemic stroke with HSV-2 infection, the high level of HSV-2 Ig G can increase the risk of adverse prognosis 14 days after the disease (or discharge), and with the increase of HSV-2 Ig G level, the risk of adverse prognosis increased.
