【摘要】：患者女,63歲。左側(cè)上顎部口腔黏膜褐色斑片3年,發(fā)生結(jié)節(jié)伴破潰6月余。皮膚科情況:左側(cè)上顎部見一邊界不清且不規(guī)則的淡褐色斑片,表面見一膚色結(jié)節(jié)。皮損組織病理示:黏膜上皮不規(guī)則增生,基底層黑素顆粒增多,真皮內(nèi)交織排列的梭形細胞呈彌漫性增生,細胞核有異型改變,病變內(nèi)散在色素顆粒。免疫組化檢查示:Vimentin和S-100彌漫陽性,Ki-67約25%陽性。診斷:口腔黏膜梭形細胞黑素瘤。治療:病變擴大手術(shù)切除,配合免疫療法。
[Abstract]:The patient was 63 years old. The left maxillary oral mucosal brown patch occurred for 3 years, and the nodules with rupture occurred more than 6 months. Dermatology: an irregular pale brown patch on the left maxilla and a pigmented nodule on the surface. The histopathology of the lesions showed irregular proliferation of mucosal epithelium, increase of melanin granules in basal layer, diffuse proliferation of fusiform cells arranged in dermis, abnormal changes of nucleus and scattered pigment granules in the lesions. Immunohistochemical examination showed that Vimentin and S-100 were diffusely positive and Ki-67 was positive in about 25%. Diagnosis: oral mucosal spindle cell melanoma. Treatment: extensive surgical resection of lesions, combined with immunotherapy.
