【摘要】：報(bào)告1例同時(shí)具有特應(yīng)性皮炎(AD)樣及肥厚性扁平苔蘚樣皮損的慢性移植物抗宿主病(cGVHD)�；颊吣�,22歲。因全身扁平丘疹伴皮膚干燥、瘙癢4個(gè)月就診。2014年4月患者因急性淋巴細(xì)胞白血病行父供子人類白細(xì)胞抗原(HLA)7/10相合異基因造血干細(xì)胞移植術(shù)。皮膚科檢查:全身皮膚干燥,毛發(fā)稀疏,面部色素沉著,軀干及四肢皮膚毛周隆起,散在紫紅色肥厚性丘疹,口腔黏膜損害,甲板縱嵴。皮損組織病理檢查:(背部結(jié)節(jié))表皮角化亢進(jìn),角質(zhì)層可見少量炎性細(xì)胞浸潤(rùn),顆粒層及棘層不規(guī)則增厚,部分區(qū)域表皮突杵狀下延,部分區(qū)域基底層液化變性;真皮淺層帶狀淋巴細(xì)胞浸潤(rùn),可見噬黑素細(xì)胞,真皮小血管增生,內(nèi)皮細(xì)胞腫脹。直接免疫熒光(-)。診斷:同時(shí)具有AD樣及肥厚性扁平苔蘚樣皮損的cGVHD。
[Abstract]:A case of chronic graft-versus-host disease (cGVHD).) with atopic dermatitis and hypertrophic lichen planus lesions at the same time is reported. The patient was 22 years old. Patients with systemic flat papules with dry skin and pruritus for 4 months were treated with donor (HLA) 7 / 10 allogeneic hematopoietic stem cell transplantation for acute lymphoblastic leukemia in April 2014. Dermatology examination: dry skin, sparse hair, pigmentation of the face, skin around the trunk and limbs, scattered in purplish red hypertrophic papules, oral mucosal damage, deck longitudinal ridge. Histopathological examination of skin lesions: (back nodules) hyperkeratosis of epidermis, keratinocyte infiltration in keratinocytes, irregular thickening of granular layer and spinous layer, extension of pestle shape in part of epidermis, liquefaction of basal layer in part of area; The superficial layer of dermis was infiltrated with lymphocytes, melanocytes, microvascular hyperplasia and endothelial cells swelling. Direct immunofluorescence (-). Diagnosis: cGVHD. with both AD and hypertrophic lichen planus lesions
