IL-20在扁平苔蘚發(fā)病及惡化過程中的意義

【摘要】：扁平苔蘚（Lichen planus，LP）為一種臨床常見的慢性或亞急性炎癥性皮膚疾病，但是目前對該病的研究并不充分，特別是對其發(fā)病機(jī)制以及在惡化過程中的相關(guān)因素研究較少。一般多認(rèn)為LP為自身免疫性疾病，因此，本研究通過對患者IL-20在患者患病及治療過程中的表達(dá),以確認(rèn)IL-20這種免疫因子對于LP進(jìn)展中的影響。 目的：分析扁平苔蘚發(fā)病過程中，IL-20在患者外周血表達(dá)水平變化特點(diǎn)，為該病的發(fā)病機(jī)理及進(jìn)展期治療提供有力的支持，從而完善該病的治療體系。同時(shí)，為進(jìn)一步的確定該病的發(fā)病機(jī)制提供有力的臨床依據(jù)。 方法：回顧性分析本院門診和住院的30例臨床確診為LP的患者的臨床治療情況，其中，有10例患者經(jīng)過臨床治療，，LP的癥狀明顯減輕，另有10例患者并無明顯的改善，另有10例患者，癥狀呈明顯的惡化趨勢。為考察上述患者治療過程中的免疫學(xué)表達(dá)特點(diǎn)，以ELISA試劑盒測定患者外周者血的IL-20的濃度。并進(jìn)行統(tǒng)計(jì)學(xué)分析，以確定IL-20在患者體內(nèi)外周血表達(dá)水平及其相關(guān)性。 結(jié)果：通過對患者的IL-20在患者體內(nèi)外周血表達(dá)水平及其變化特點(diǎn)可知，全部30例患者在治療初期IL-20的表達(dá)情況均明顯高于正常人外周血。并且，通過一個(gè)治療周期后發(fā)現(xiàn)，LP癥狀減輕的患者，其IL-20的表達(dá)有顯著性下降，但是仍然高于正常人外周血；LP癥狀惡化的患者，其IL-20的表達(dá)并無顯著變化，該結(jié)果表明， IL-20在患者發(fā)生扁平苔蘚的過程中，起到了顯著性的作用。但是，在進(jìn)一步惡化過程中，IL-20并非主要因素。 結(jié)論：IL-20在患者LP發(fā)病過程中起到了顯著性作用，此參數(shù)的提高可以大大增加患者LP發(fā)病的風(fēng)險(xiǎn)；而在進(jìn)一步的惡化過程中， IL-20的影響并不大，LP癥狀減輕的患者，其IL-20的表達(dá)有顯著性下降。從而確定，IL-20同患者的發(fā)病關(guān)系非常密切。
[Abstract]:Lichen planus (Lichen planus,LP) is a common chronic or subacute inflammatory skin disease, but it is not well studied at present, especially on its pathogenesis and related factors in the process of deterioration. LP is generally considered to be an autoimmune disease. Therefore, the expression of IL-20 in patients during the course of disease and treatment was studied in order to confirm the influence of IL-20, an immune factor, on the progression of LP. Objective: to analyze the changes of IL-20 expression in peripheral blood of patients with lichen planus (Lichen planus), and to provide strong support for the pathogenesis and advanced treatment of Lichen planus, so as to improve the therapeutic system of Lichen planus. At the same time, to further determine the pathogenesis of the disease to provide a strong clinical basis. Methods: the clinical treatment of 30 patients with LP was analyzed retrospectively. Among them, 10 patients with LP were obviously relieved after clinical treatment, and 10 patients had no significant improvement. In the other 10 patients, the symptoms showed an obvious trend of deterioration. In order to investigate the immunological expression characteristics of the patients, the concentration of IL-20 in peripheral blood was measured by ELISA kit. Statistical analysis was carried out to determine the expression of IL-20 in peripheral blood of patients and its correlation. Results: the expression of IL-20 in peripheral blood of all 30 patients was significantly higher than that of normal controls at the beginning of treatment. After a treatment cycle, the expression of IL-20 in patients with mild LP symptoms was significantly decreased, but the expression of IL-20 was still higher than that in patients with normal peripheral blood and LP symptoms, and there was no significant change in the expression of IL-20. The results suggest that IL-20 plays a significant role in the development of lichen planus. However, IL-20 is not a major factor in the process of further deterioration. ConclusionTwo -IL-20 plays a significant role in the pathogenesis of LP, and the increase of this parameter can greatly increase the risk of LP, but in the process of further deterioration, the effect of IL-20 is not significant in patients with lessening of LP symptoms. The expression of IL-20 decreased significantly. Thus it is determined that IL-20 is closely related to the onset of the disease.
