【摘要】：非器官樣型表皮痣,簡(jiǎn)稱表皮痣,又稱疣狀痣,是表皮角質(zhì)形成細(xì)胞異常增殖形成的一種皮膚錯(cuò)構(gòu)瘤,表現(xiàn)為局限或泛發(fā)的集簇性分布的膚色、棕褐色或者灰褐色的疣狀丘疹,皮損可融合成邊界清楚的乳頭瘤樣斑塊,四肢的皮損大多沿Blaschko線分布。根據(jù)臨床特點(diǎn)及皮損范圍可將表皮痣分為三個(gè)亞型：局限型、泛發(fā)型和炎性線狀表皮痣。本病多散發(fā),罕見家族發(fā)病者�，F(xiàn)報(bào)告3例有家族史的表皮痣患者并復(fù)習(xí)相關(guān)文獻(xiàn)�；颊逜,男,22歲,出生后10天左右肛周皮膚出現(xiàn)綠豆粒大小疣狀高起皮疹,之后逐年增大、增多,發(fā)展為全身多處疣狀密集的褐紅色丘疹,部分融合成斑塊,呈帶狀分布。自感肛周皮損瘙癢難忍,便后尤甚。皮損活檢示：表皮角化過度、角化不全,棘層增生肥厚,皮突下延交織,基底細(xì)胞色素增加；真皮淺層血管擴(kuò)張,單一核細(xì)胞灶狀浸潤(rùn)。結(jié)合臨床診斷：泛發(fā)性炎性表皮痣�；颊連,男,2歲,其母為A的表姐,自出生肛周即出現(xiàn)與A類似皮損。皮損活檢示與A類似�；颊逤,女,系A(chǔ)的母親,右側(cè)鼻唇溝處局限性融合性黃白色角化性丘疹,自幼即有。診斷：局限型表皮痣。該家族中共3代5例罹患此病。本文通過對(duì)相關(guān)文獻(xiàn)復(fù)習(xí),綜述了表皮痣的流行病學(xué)特點(diǎn)、病因及發(fā)病機(jī)制、臨床分型、組織病理學(xué)特點(diǎn)、鑒別診斷及治療方法。
[Abstract]:Non-organ-like epidermal nevus, also called verrucous nevus, is a skin hamartoma formed by abnormal proliferation of epidermal keratinocytes. It is characterized by localized or generalized clustered skin color, brown or grayish-brown verrucous papules, and the lesions can fuse into well-defined papillomatous plaques. Most of the skin lesions of the extremities are along Blas. The epidermal nevus can be divided into three subtypes according to the clinical features and lesion extent: localized, generalized and inflammatory linear nevus. The disease is more sporadic and rarely occurs in a family. Three cases of epidermal nevus with family history were reported and the relevant literature was reviewed. Patient A, male, 22 years old, presented with large mung bean granules on the perianal skin about 10 days after birth. The skin lesion biopsy showed that the epidermis was hyperkeratosis, incomplete keratosis, hypertrophy of the spinous layer, interlaced subcutaneous processes and increased basal cytochrome. Superficial dermal vasodilation with focal mononuclear infiltration. Combined with clinical diagnosis: generalized inflammatory epidermal nevus. Patient B, male, 2 years old, mother of A's cousin, had a skin lesion similar to A since birth around the anus. Skin biopsy showed a similar lesion to A. Patient C, female, mother of A, localized fusion of yellowish-white keratotic papules at the right nasolabial groove, i.e. at an early age. Diagnosis: localized nevus of the epidermis. There are 5 cases of this disease in this family of three generations. This paper reviews the epidemiological characteristics, etiology and pathogenesis, clinical classification, histopathological characteristics, differential diagnosis and treatment of epidermal nevus.
