【摘要】：目的：人類皰疹病毒8型在自然人群特別是新疆少數(shù)民族中有較高的感染率，探討免疫組織化學(xué)方法檢測(cè)Kaposi肉瘤中人類皰疹病毒8型的可行性及其診斷意義。方法：采用免疫組織化學(xué)技術(shù)分別檢測(cè)58例Kaposi肉瘤組織斑片期、結(jié)節(jié)期和40例化膿性肉芽腫組織中HHV-8的表達(dá)，檢測(cè)結(jié)果采用卡方檢驗(yàn)四格表Fisher確切概率法分析Kaposi肉瘤組織斑片期、結(jié)節(jié)期及化膿性肉芽腫組織中HHV-8表達(dá)的差異。結(jié)果：1.HHV-8在Kaposi肉瘤真皮瘤體中的表達(dá)陽(yáng)性率為94.83%(55/58)，在化膿性肉芽腫中的表達(dá)均為陰性，在Kaposi肉瘤組織和血管瘤組織瘤體中表達(dá)在統(tǒng)計(jì)學(xué)有顯著性差異（P＜0.05）。2.HHV-8在Kaposi肉瘤組織表皮中表達(dá)陽(yáng)性率為6.90%（4/58），血管內(nèi)皮細(xì)胞中的表達(dá)陽(yáng)性率為91.38%(53/58)，真皮梭形細(xì)胞中表達(dá)陽(yáng)性率94.83%(55/58)。3.結(jié)節(jié)期Kaposi肉瘤中表達(dá)HHV-8陽(yáng)性的細(xì)胞所占比例較斑片期及斑塊期明顯增多，，其差異有統(tǒng)計(jì)學(xué)意義（P0.05）。結(jié)論：免疫組織化學(xué)方法檢測(cè)人類皰疹病毒8型在Kaposi肉瘤的診斷尤其是鑒別診斷中具有重要意義。
[Abstract]:Objective: human herpesvirus 8 has a high infection rate in natural population, especially in Xinjiang ethnic minorities. To explore the feasibility and diagnostic significance of immunohistochemical method for detection of human herpesvirus 8 in Kaposi sarcoma. Methods: immunohistochemical technique was used to detect the expression of HHV-8 in 58 cases of Kaposi's sarcoma at macular stage, nodular stage and 40 cases of suppurative granuloma. Results the difference of HHV-8 expression in Kaposi sarcoma tissue in macular stage, nodular stage and suppurative granuloma was analyzed by chi-square test four grid table Fisher exact probability method. Results 1. The positive rate of HHV-8 was 94.83% (55 / 58) in Kaposi sarcoma dermis, and negative in suppurative granuloma. The positive rate of HHV-8 expression was 6.90% (4 / 58) in epidermis of Kaposi sarcoma, 91.38% (53 / 58) in vascular endothelial cells and 94.83% (55 / 58) in dermal fusiform cells. The proportion of HHV-8 positive cells in nodular Kaposi sarcoma was significantly higher than that in plaque stage (P0.05). Conclusion: the detection of human herpesvirus 8 by immunohistochemical method is of great significance in the diagnosis of Kaposi sarcoma, especially in differential diagnosis.
