【摘要】： 目的 探討三種常用洗發(fā)香波1%二硫化硒、0.5%吡羅克酮乙醇胺和1%吡硫翁鋅治療頭皮屑的療效、作用和安全性。 材料與方法 采用隨機(jī)、雙盲、陽(yáng)性藥物對(duì)照試驗(yàn),納入132例中、重度頭皮屑患者,比較1%二硫化硒、0.5%吡羅克酮乙醇胺和1%吡硫翁鋅治療時(shí)頭皮屑臨床嚴(yán)重度評(píng)分、馬拉色菌計(jì)數(shù)、相關(guān)細(xì)胞因子IL-1α/TP、IL-8/TP和TNF-α/TP的變化及其關(guān)系。 結(jié)果 三種香波使用過(guò)程中均無(wú)不良反應(yīng)發(fā)生。在4周的治療期后,三組頭皮屑臨床嚴(yán)重度評(píng)分和馬拉色菌計(jì)數(shù)均較基線明顯降低(P0.001),三組間的差異均無(wú)統(tǒng)計(jì)學(xué)意義。在治療結(jié)束后的隨訪期各組臨床嚴(yán)重度評(píng)分及馬拉色菌計(jì)數(shù)皆有增加(P0.001),1%吡硫翁鋅組臨床嚴(yán)重度評(píng)分的增加較其余兩組為重(P0.001),而馬拉色菌計(jì)數(shù)的增加在三組之間無(wú)統(tǒng)計(jì)學(xué)差異。治療期間細(xì)胞因子IL-1α/TP、IL-8/TP和TNF-α/TP均明顯降低(P0.001)；在隨訪期,IL-1α/TP有顯著上升(P0.05),而IL-8/TP和TNF-α/TP無(wú)明顯改變,三組之間各細(xì)胞因子變化稍有不同。 結(jié)論 1%二硫化硒、0.5%吡羅克酮乙醇胺和1%吡硫翁鋅三種洗發(fā)香波安全性良好,對(duì)頭皮屑均有較好的治療效果,而停止使用時(shí)病情會(huì)有所恢復(fù)。三種香波對(duì)頭皮屑臨床嚴(yán)重度評(píng)分和馬拉色菌的改善相似,但1%二硫化硒及0.5%吡羅克酮乙醇胺較1%吡硫翁鋅對(duì)臨床癥狀的改善更為持久。IL-1α能夠較IL-8和TNF-α更靈敏地反映頭皮屑嚴(yán)重程度變化。
[Abstract]:Objective to investigate the efficacy, effect and safety of three kinds of shampoo, 1% selenium disulfide 0.5% piroclone ethanolamine and 1% pyrithion zinc in the treatment of dandruff. Materials and methods 132 patients with severe dandruff were enrolled in a randomized, double-blind, positive drug controlled trial. The clinical severity score of dandruff, Malassezia count, IL-1 偽 / TPIL-8TP, TNF- 偽 -p-TP and TNF- 偽 -p-TP were compared between 1% selenite 0.5% piroxone ethanolamine and 1% pyrithion zinc. Results there was no adverse reaction during the use of three kinds of shampoo. After 4 weeks of treatment, the clinical severity score and Malassezia count of dandruff in the three groups were significantly lower than those in the baseline (P0.001), and there was no significant difference among the three groups. The clinical severity score and Malassezia count of each group were increased (P0.001) in the follow-up period after treatment compared with the other two groups (P0.001), but there was no significant difference between the three groups in the increase of Malassezia count. During the treatment period, the cytokines IL-1 偽 / TPt8 / TP and TNF- 偽 / TP decreased significantly (P0.001), but IL-8 / TP and TNF- 偽 / TP did not change significantly during the follow-up period (P0.05), and the changes of cytokines were slightly different among the three groups. Conclusion 1% selenite sulphide 0.5% piroxone ethanolamine and 1% pyrithion zinc shampoo are safe and effective in the treatment of dandruff. The effect of three kinds of shampoo on dandruff clinical severity score was similar to that of Malassezia. However, 1% selenium disulfide and 0.5% piroxone ethanolamine could more sensitively reflect the severity of dandruff than that of 1% zinc pyrithion. IL-1 偽 could reflect the severity of dandruff more sensitively than IL-8 and TNF- 偽.
