【摘要】：目的探討外毛根鞘癌(trichilemmal carcinoma,TC)的臨床病理學(xué)特征、診斷及鑒別診斷。方法回顧性分析7例TC患者的臨床資料、組織學(xué)形態(tài)及免疫表型,并復(fù)習(xí)相關(guān)文獻(xiàn)。結(jié)果 7例患者年齡22~63歲,平均40.5歲,其中男性4例,女性3例。皮損均發(fā)生于頭面部,表現(xiàn)為孤立的外生性團(tuán)塊。鏡檢:腫瘤組織呈分葉狀、索條狀,主要由不典型的透明細(xì)胞組成,以伴有外毛根鞘角化的上皮細(xì)胞增生為特征。免疫表型:7例PAS染色,透明細(xì)胞均(+),CK5和CK14均(+),CEA、S-100和HMB-45均(-)。7例患者均行手術(shù)切除術(shù)。6例患者隨訪1~5年,無復(fù)發(fā)及轉(zhuǎn)移。結(jié)論TC是低度惡性的附屬器腫瘤,具有局部侵襲性、基本不復(fù)發(fā)、轉(zhuǎn)移的特點(diǎn)。診斷依據(jù)分葉狀結(jié)構(gòu)及外毛根鞘分化特點(diǎn)。手術(shù)切除,預(yù)后好。需與外毛根鞘瘤及其他透明細(xì)胞腫瘤鑒別。
[Abstract]:Objective to investigate the clinicopathological features, diagnosis and differential diagnosis of trichilemmal carcinoma. Methods the clinical data, histological morphology and immunophenotype of 7 patients with TC were analyzed retrospectively. Results the age of 7 patients was 22 ~ 63 years (mean 40.5 years), including 4 males and 3 females. Skin lesions occur on the head and face, showing an isolated ectogenic mass. Microscopic examination: tumor tissue is lobulated, cord-like, mainly composed of atypical transparent cells, characterized by the proliferation of epithelial cells with keratosis of outer hair root sheath. Immunophenotype: pas staining was performed in 7 cases, and clear cells were (1) CK5 and CK14 () CEAS-100 and HMB-45 (-). 6 cases were followed up for 1 ~ 5 years without recurrence or metastasis. Conclusion TC is a low grade malignant adnexal tumor with local invasiveness, no recurrence and metastasis. The diagnosis was based on lobular structure and differentiation of outer hair root sheath. Surgical resection has a good prognosis. It should be distinguished from outer hair root sheath tumor and other clear cell tumors.
