【摘要】：目的探討神經(jīng)梅毒的臨床表現(xiàn)、實驗室檢查和影像學檢查特征。方法回顧性分析18例神經(jīng)梅毒患者的臨床表現(xiàn)、實驗室檢查和影像學檢查資料。結果 18例神經(jīng)梅毒患者平均發(fā)病年齡53.83歲,其中主質型8例(44.44%),其中麻痹性癡呆4例,脊髓癆3例,混合型1例;腦膜血管梅毒4例(22.22%);無癥狀型3例(16.67%);樹膠樣腫型2例(11.11%);腦膜炎型1例(5.56%)。血清血漿快速反應素試驗(RPR)、梅毒螺旋體顆粒凝集試驗(TPPA)檢測均為陽性。腦脊液檢查:18例患者TPPA均陽性,9例RPR陽性,9例腦脊液白細胞數(shù)≥10×106/L,7例腦脊液蛋白0.50g/L。影像學表現(xiàn)為:腦梗死,腦萎縮,腦白質變性,額葉、顳葉、海馬區(qū)異常信號及大腦后動脈閉塞,顱內動脈硬化。結論神經(jīng)梅毒表現(xiàn)多樣,臨床體征、血清學、腦脊液及影像學對于診斷神經(jīng)梅毒及分型有重要指導意義。
[Abstract]:Objective to investigate the clinical manifestations, laboratory and imaging features of neurosyphilis. Methods 18 cases of neurosyphilis were analyzed retrospectively. Results the average age of 18 patients with neurosyphilis was 53.83 years old. Among them, 8 cases had primary type of neurosyphilis (4 cases of paralytic dementia, 3 cases of tuberculosis and 1 case of mixed type). Meningeal vascular syphilis (n = 4), asymptomatic type (n = 3, n = 16), gum type (n = 2, n = 11) and meningitis type (n = 1, n = 5.56). Serum rapid response hormone test (RPRP) and Treponema pallidum agglutination test (TPPA) were all positive. Cerebrospinal fluid (CSF) examination showed TPPA positive in 9 cases and RPR positive in 9 cases. WBC count in cerebrospinal fluid was 鈮，
