人乳頭瘤病毒所致皮膚疣的病毒檢測與分型

【摘要】:背景: 人乳頭瘤病毒(human papillomavirus HPV)可引起皮膚和粘膜良性增生,導(dǎo)致各種皮膚疣的發(fā)生,可引起生殖器皮膚黏膜惡性疾病。不同HPV亞型所引起的疾病種類也各不相同。而且HPV感染型別有種族和地域差異性。 目的: 研究本地患者HPV感染皮損標(biāo)本中HPV亞型的分布。分析,評價聚合酶鏈反應(yīng)(PCR)-反向點(diǎn)雜交法(reverse dot blot)檢測HPV亞型的靈敏度、準(zhǔn)確性和臨床應(yīng)用價值,為下一步建立HPV感染動物模型,進(jìn)行藥物篩選工作打下基礎(chǔ)。 方法: 采用PCR-反向點(diǎn)雜交法對2011年10月至2012年4月在我院皮膚科就診的40例尋常疣和尖銳濕疣標(biāo)本進(jìn)行HPV DNA的基因分型檢測。其中15例尋常疣標(biāo)本,25例尖銳濕疣標(biāo)本。年齡12-70歲。 結(jié)果: 在15例尋常疣標(biāo)本中,HPV的檢出例數(shù)為11例,檢出率為73.3%。檢測出HPV型別分別為:HPV1型7例,占總檢出例數(shù)的63.6%;HPV2型2例,占18.2%,HPV1,,2型混合感染2例,占18.2%。 在25例尖銳濕疣標(biāo)本中,HPV的檢出例數(shù)為23例,檢出率為92%。檢測出HPV型別分別為:單一型感染14例,占60.9%,其中HPV6型5例,占21.7%;HPV11型9例,占39.1%;旌细腥9例,占39.1%。其中HPV6,11型混合感染3例,占13%;HPV6,16型混合感染2例,占8.7%;HPV6,18型2例,占8.7%;HPV11,52型1例,占4.3%;HPV11,53型1例,占4.3%。 結(jié)論: 共檢出2種HPV亞型與尋常疣有關(guān),6種亞型與尖銳濕疣有關(guān)。尋常疣中HPV1型感染率最高,達(dá)63.6%;尖銳濕疣中HPV11型感染率最高,達(dá)39.1%。我們還發(fā)現(xiàn)了尖銳濕疣中存在HPV16,18,52,53四種高危型別混合感染。 結(jié)果顯示,PCR-反向點(diǎn)雜交法檢測皮膚疣標(biāo)本中HPV亞型敏感性高,檢出率高,適合臨床應(yīng)用。
[Abstract]:Background: Human papillomavirus (HPV) can cause benign hyperplasia of skin and mucosa, lead to the occurrence of various skin warts and lead to malignant diseases of genital skin and mucosa. The types of diseases caused by different HPV subtypes are also different. And there are ethnic and regional differences in the type of HPV infection. Objective: To study the distribution of HPV subtypes in skin lesions of local patients with HPV infection. To evaluate the sensitivity, accuracy and clinical value of polymerase chain reaction-reverse dot hybridization (PCR) for detecting HPV subtypes, and to lay a foundation for the establishment of animal model of HPV infection and drug screening. Methods: HPV DNA genotyping of 40 specimens of verruca vulgaris and condyloma acuminatum from October 2011 to April 2012 was detected by PCR- reverse dot hybridization. 15 cases of verruca vulgaris were collected from 25 cases of condyloma acuminatum. Age 12-70. Results: Among 15 specimens of verruca vulgaris, 11 cases were detected and the detection rate was 73.3%. There were 7 cases of HPV type 1, accounting for 63.6 cases of HPV2 type, and 2 cases of mixed infection of HPV1 type 2 cases (18.2%). In 25 cases of condyloma acuminatum, 23 cases were detected and the detection rate was 92%. There were 14 cases of single type infection (60.9%), 5 cases of HPV6 type, 9 cases of HPV11 type (39.1%). Mixed infection was found in 9 cases (39.1%). Among them, there were 3 cases of mixed infection of HPV6 / 11 type, 2 cases of mixed infection of HPV6 / 16 type, 2 cases of HPV618 type, 1 case of HPV1152 type, 1 case of HPV1153 type, and 1 case of HPV1153 type. Conclusion: Two HPV subtypes were found to be associated with verruca vulgaris and 6 subtypes were associated with condyloma acuminatum. The infection rate of HPV1 type was the highest in verruca vulgaris (63.6%), and the infection rate of HPV11 type in condyloma acuminatum was the highest (39.1%). We also found four high risk types of mixed infection in condyloma acuminatum. The results showed that PCR- reverse dot hybridization was suitable for clinical application in the detection of HPV subtypes in patients with verruca skin.
