【摘要】：目的檢測(cè)尋常型銀屑病患者血清中IL-17、IL-35的表達(dá)水平,分析它們?cè)阢y屑病進(jìn)行期和靜止期的變化規(guī)律及其與PASI評(píng)分的相關(guān)性,進(jìn)一步探討它們?cè)阢y屑病發(fā)病中的作用,為未來銀屑病的治療提供依據(jù)。方法1、研究對(duì)象:收集我院皮膚科門診2016年2月至2016年11月收治的尋常型銀屑病患者共29例。入選標(biāo)準(zhǔn):(1)此類患者有明顯的尋常型銀屑病樣皮損。(2)排除皮肌炎、天皰瘡、慢性蕁麻疹、惡性腫瘤、紅斑狼瘡等一系列自身免疫性疾病以及有其他合并癥的患者。(3)該患者3個(gè)月內(nèi)未使用過免疫抑制劑、維A酸類、類固醇激素等能影響自身免疫功能的藥物。根據(jù)患者的病情將病例組分為兩組:進(jìn)行期組和靜止期組。分別為16例和13例。由同一位醫(yī)務(wù)人員對(duì)銀屑病患者進(jìn)行相應(yīng)的PASI評(píng)分并做記錄。健康對(duì)照組29例均來自我院體檢中心的健康人員,且他們無自身免疫性疾病及相應(yīng)的家族史,經(jīng)t檢驗(yàn)后,病例組和健康對(duì)照組在性別和年齡方面相比,差異無統(tǒng)計(jì)學(xué)意義。2、方法:采用雙抗體夾心酶聯(lián)免疫的方法(ELISA)對(duì)銀屑病組和健康對(duì)照組人員血清中IL-17和IL-35的表達(dá)水平進(jìn)行檢測(cè)并做記錄。3、統(tǒng)計(jì):用SPSS 21.0軟件對(duì)本實(shí)驗(yàn)所得的數(shù)據(jù)進(jìn)行統(tǒng)計(jì)學(xué)處理。計(jì)量資料用均值±標(biāo)準(zhǔn)差(x±s)表示。計(jì)量資料兩組間均數(shù)比較可用t檢驗(yàn);多組間則可用單因素方差分析(方差齊的用LSD法,方差不齊的用DunnettT3法)。分類變量則采用百分比表示,比較采用卡方檢驗(yàn)。兩個(gè)連續(xù)型隨機(jī)變量的相關(guān)性用Pearson相關(guān)分析。P0.05為差異具有統(tǒng)計(jì)學(xué)意義。結(jié)果1、尋常型銀屑病患者血清中IL-17表達(dá)水平高于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);其中,進(jìn)行期血清中IL-17表達(dá)水平高于靜止期,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。2、尋常型銀屑病患者血清中IL-35表達(dá)水平低于健康對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P0.05);其中,進(jìn)行期血清中IL-35的表達(dá)水平低于靜止期,差異有統(tǒng)計(jì)學(xué)意義(P0.05)。3、尋常型銀屑病患者血清中IL-17與IL-35表達(dá)水平呈負(fù)相關(guān)(P0.05)。4、尋常型銀屑病患者血清中IL-17表達(dá)水平與PASI評(píng)分呈正相關(guān)(P0.05);尋常型銀屑病患者血清中IL-35表達(dá)水平與PASI評(píng)分呈負(fù)相關(guān)(P0.05)。結(jié)論1、IL-17可能作為一種促炎因子參與了銀屑病的發(fā)病;IL-35作為一種保護(hù)因子參與了銀屑病的發(fā)病;且它們的表達(dá)水平可能與銀屑病的嚴(yán)重程度有關(guān)。2、IL-17、IL-35失衡可能參與了銀屑病的發(fā)病和免疫應(yīng)答的過表達(dá)。
[Abstract]:Objective to detect the expression of IL-17 and IL-35 in serum of patients with psoriasis vulgaris, to analyze the changes of IL-17 IL-35 in psoriasis and its correlation with PASI score, and to explore the role of IL-17 IL-35 in the pathogenesis of psoriasis. To provide the basis for the treatment of psoriasis in the future. Methods 1. Participants: 29 patients with psoriasis vulgaris were collected from February 2016 to November 2016. Admission criteria: 1) these patients have obvious psoriasis vulgaris lesions.) exclude dermatomyositis, pemphigus, chronic urticaria, malignant tumor, A series of autoimmune diseases such as lupus erythematosus and other complications.) the patient has not used immunosuppressants, retinoic acids, steroids and other drugs that affect autoimmune function in 3 months. The patients were divided into two groups according to the patient's condition: the progressive group and the rest group. There were 16 cases and 13 cases respectively. The PASI score of psoriasis patients was recorded by the same medical staff. All 29 healthy persons in the healthy control group were from the physical examination center of our hospital, and they had no autoimmune disease and corresponding family history. After t test, the sex and age of the case group and the healthy control group were compared. Methods: the expression of IL-17 and IL-35 in the serum of psoriasis group and healthy control group were detected by double antibody sandwich enzyme-linked immunosorbent assay (Elisa) and recorded. 3. Statistics: SPSS 21.0 software was used to detect the expression of IL-35 and IL-17. The experimental data were processed statistically. The measurement data were expressed as mean 鹵standard deviation (x 鹵s). T test can be used to compare the mean of measurement data between two groups, and single factor analysis of variance (LSD method is used for homogeneous variance, DunnettT3 method is used for uneven variance). Classification variables were expressed as percentages, and chi-square tests were used to compare them. The correlation of two continuous random variables was statistically significant with Pearson correlation analysis. Results 1. The expression of IL-17 in patients with psoriasis vulgaris was higher than that in healthy controls (P 0.05), and the expression of IL-17 in patients with psoriasis vulgaris was higher than that in patients with psoriasis. The expression level of IL-35 in patients with psoriasis vulgaris was lower than that in healthy controls, and the difference was statistically significant (P 0.05), among which, the expression of IL-35 in the serum of patients with psoriasis vulgaris was lower than that in the stationary stage. The expression of IL-17 and IL-35 in patients with psoriasis vulgaris was negatively correlated with that in patients with psoriasis vulgaris. The expression of IL-17 in serum of patients with psoriasis vulgaris was positively correlated with the PASI score (P0.05), and the IL-35 table in serum of patients with psoriasis vulgaris was positively correlated with that of the patients with psoriasis vulgaris. There was a negative correlation between PASI score and reach level (P 0.05). Conclusion 1IL-17 may be a proinflammatory factor involved in the pathogenesis of psoriasis. IL-35, as a protective factor, may participate in the pathogenesis of psoriasis. Their expression level may be related to the severity of psoriasis. The imbalance of IL-17 and IL-35 may be involved in the pathogenesis of psoriasis and the overexpression of immune response.
