IL-17和IL-23在濕疹患者中的表達及其臨床意義

【摘要】:目的:濕疹是一種是由多種內(nèi)外因素引起的變態(tài)反應性疾病,發(fā)病機制復雜,原因不明,極度瘙癢且易反復發(fā)作。以往認為在濕疹的發(fā)病中Th2占優(yōu)勢的Th1/Th2細胞亞群的失衡是其發(fā)病的一個主要機制,但此失衡理論并不能完全解釋其發(fā)病機制。新近研究發(fā)現(xiàn),機體存在一種新型的、以分泌IL-17為主要特征的細胞群,其在分化、生物學作用等方面不同于1型和2型T細胞,被命名為Th17細胞。IL-23也是近年新發(fā)現(xiàn)的細胞因子,與IL-12共同分享IL-12p40和IL-12Rβ1亞單位。研究發(fā)現(xiàn),IL-23主要作用于記憶CD4+T細胞,誘導Th1型細胞的分化及IFN-γ產(chǎn)生,并發(fā)現(xiàn)其還可作用于記憶Th17細胞亞群誘導產(chǎn)生IL-17,對于Th17細胞的生存和活性有重要的輔助作用。因此Th17細胞亞群和IL-17、IL-23的發(fā)現(xiàn)為人們重新認識免疫應答和調(diào)節(jié)機制提供了方向。本實驗主要研究IL-17和IL-23在濕疹患者和正正常人外周血中的表達及其差異,初步探討1L-17和IL-23在濕疹發(fā)病機制中的作用和臨床意義。 方法:參照趙辯主編的臨床皮膚病學診斷標準和相關文獻選取45例濕疹患者,男23例,女22例,年齡16—78歲,平均(35.68±10.58)歲。按照臨床表現(xiàn)將患者進行分期,其中急性期患者15例,亞急性期15例,慢性期15例。以性別和年齡相匹配的30例健康體檢者(男14例,女16例,年齡18—76歲,平均37.22±8.56歲)作為對照組。采用ELISA方法檢測血清中IL-17和IL-23的表達水平,并收集整理濕疹患者的臨床資料及實驗室數(shù)據(jù)。 結(jié)果: 1.急性期和亞急性期濕疹患者血清中IL-17水平明顯高于慢性期患者和正正常組(p0.05),但IL-17水平在急性期和亞急性期、慢性期和正正常組間則無統(tǒng)計學差異。 2.急性期和亞急性期濕疹患者血清中IL-23水平明顯高于慢性期患者和正正常組(p0.05),但IL-23水平在急性期和亞急性期、慢性期和正正常組間則無統(tǒng)計學差異。 3.濕疹患者血清中1L-17與IL-23表達水平呈正直線相關(r=0.53,P0.05)關系。 4.濕疹患者血清中IL-17和IL-23水平與年齡、血沉、白蛋白、球蛋白、血肌酐、淋巴細胞、嗜酸性粒細胞計數(shù)和血小板計數(shù)之間無明顯相關關系。 結(jié)論:IL-17和IL-23在濕疹患者血清中表達水平增高,且急性期和亞急性期更為明顯,具有統(tǒng)計學差異。提示IL-17和IL-23可能參與了濕疹疾病的病理過程,并且與患者疾病的嚴重程度有關系。

Research found, IL-23 It mainly acts on the memory of CD4+T cells, induces the differentiation of Th1 type cells and the production of IFN- gamma. It is found that it can also play an important role in the survival and activity of Th17 cells, which can also play an important role in the survival and activity of Th17 cells. Therefore, the discovery of Th17 cell subsets and IL-17 and the discovery of IL-23 for people to reunderstand the immune response and regulation mechanism This experiment mainly studies the expression and difference of IL-17 and IL-23 in eczema patients and normal human peripheral blood, and preliminarily discusses the role and clinical significance of 1L-17 and IL-23 in the pathogenesis of eczema.
Methods: according to the clinical dermatology diagnostic criteria and related literature, 45 cases of eczema, 23 male and 22 female, with an average age of 16 to 78 years, average (35.68 + 10.58) years, were divided according to clinical manifestations, including 15 in acute stage, 15 in subacute stage, 15 in chronic period, and 30 health with sex and age.
