不良生活習(xí)性對青少年痤瘡患者Th17、Treg、sIL-2R及TNF-α表達(dá)的調(diào)控作用研究

【摘要】:目的本研究選取吸煙、飲酒、睡眠不足三類不良生活習(xí)性的青少年為研究對象,重點(diǎn)探討青少年不良習(xí)性與Th17、Treg細(xì)胞及相關(guān)細(xì)胞因子TNF-α、sIL-2R引起的免疫紊亂以及痤瘡患病的相關(guān)性,對青少年的健康成長具有重要指導(dǎo)價(jià)值。方法1、采用現(xiàn)況調(diào)查研究方法,以隨機(jī)整群抽樣方法抽取菏澤家政職業(yè)學(xué)院384名學(xué)生作為樣本,自擬問卷調(diào)查表,進(jìn)行匿名問卷調(diào)查。分析在校青少年不良習(xí)性和痤瘡患病現(xiàn)狀,采用卡方檢驗(yàn)和Logistic回歸分析探討青少年不良習(xí)性中可能影響?zhàn)畀徎疾〉奈kU(xiǎn)因素。2、從2015年5月到2015年9月菏澤家政職業(yè)學(xué)院參加健康體檢的學(xué)生中選擇60人作為研究對象,共分為三組,吸煙組20人,睡眠不足組20人,對照組20人。用酶聯(lián)免疫分析(ELISA)方法測定血清中sIL-2R、TNF-α濃度;用流式細(xì)胞術(shù)測定外周血Th17、Treg細(xì)胞比例;用RT-qPCR測定外周血TNF-α、Foxp3、RORγt mRNA水平。采用單因素方差分析和spearson相關(guān)分析法探討吸煙、睡眠不足同sIL-2R、TNF-α、Th17、Treg 之間的關(guān)系。3、從2015年10月到2016年8月在單縣中心醫(yī)院皮膚科就診的青少年痤瘡患者中,按照臨床診斷情況及Pillsbury分級法選擇60名患者,將其分別分為痤瘡Ⅰ級組、痤瘡Ⅱ級組、痤瘡Ⅲ級組、痤瘡Ⅳ級組,每組15人,并選取15名健康體檢者作為對照組。用酶聯(lián)免疫分析(ELISA)方法測定血清中sIL-2R、TNF-α濃度;用流式細(xì)胞術(shù)測定外周血Th17、Treg細(xì)胞比例。采用單因素方差分析和spearson相關(guān)分析法探討?zhàn)畀弴?yán)重程度同sIL-2R、TNF-α、Th17、Treg之間的關(guān)系。結(jié)果1、單因素分析,吸煙、睡眠不足、飲酒都是痤瘡的可疑影響因素,二元Logistic回歸分析,只有吸煙、睡眠不足是痤瘡的危險(xiǎn)影響因素。2、吸煙組、睡眠不足組同對照組相比,sIL-2R、TNF-α濃度顯著升高,差異有統(tǒng)計(jì)學(xué)意義;Th17細(xì)胞比例、Th17/Treg顯著升高,差異有統(tǒng)計(jì)學(xué)意義;TNF-α、RORγt mRNA相對表達(dá)水平顯著升高,差異有統(tǒng)計(jì)學(xué)意義。吸煙量、睡眠不足時(shí)間同 sIL-2R、TNF-α、Th17、Th17/Treg 明顯正相關(guān)。3、各痤瘡患者組與對照組相比,sIL-2R、TNF-α濃度顯著升高,差異有統(tǒng)計(jì)學(xué)意義;Th17、Th17/Treg顯著升高,差異有統(tǒng)計(jì)學(xué)意義。痤瘡嚴(yán)重程度同sIL-2R、TNF-α、Th17、Th17/Treg 明顯正相關(guān)。結(jié)論本研究從基因、分子水平證明青少年吸煙、飲酒、睡眠不足的不良習(xí)性可導(dǎo)致外周血sIL-2R、TNF-α、Th17增高,Th17/Treg比例失衡,外周血RORyt、TNF-αmRNA高表達(dá)。在痤瘡炎癥患者中這些免疫紊亂更為顯著。由此表明吸煙、睡眠不足引發(fā)的免疫失衡是導(dǎo)致痤瘡炎癥發(fā)生的主要病因。故本課題對青年學(xué)子良好生活習(xí)性的培養(yǎng)具有重要引導(dǎo)價(jià)值。
[Abstract]:Objective to investigate the relationship between unhealthy habits of adolescents, such as smoking, drinking and lack of sleep, and the immune disorder induced by Th17 Treg cells and the related cytokines TNF- 偽 -nsIL-2R, as well as the prevalence of acne, in order to explore the relationship between the unhealthy habits of adolescents and the immune disorder induced by Th17 Treg cells and the related cytokines TNF- 偽 -nsIL-2R. It has important guiding value for the healthy growth of teenagers. Methods 1. 384 students of Heze Home Economics and Vocational College were sampled by random cluster sampling method. To analyze the prevalence of bad habits and acne among adolescents in school. Chi-square test and Logistic regression analysis were used to explore the risk factors of acne prevalence in adolescents. From May 2015 to September 2015, 60 students from Heze Home Economics Vocational College were selected as subjects. They were divided into three groups: smoking group (n = 20), sleep deprivation group (n = 20) and control group (n = 20). The concentration of sIL-2RnTNF- 偽 in serum was determined by Elisa, the percentage of Th17Treg cells in peripheral blood was measured by flow cytometry, and the level of TNF- 偽 Foxp3ROR 緯 t mRNA in peripheral blood was determined by RT-qPCR. Univariate analysis of variance (ANOVA) and spearson correlation analysis were used to investigate the relationship between smoking, sleep deprivation and sIL-2RN- 偽 TNF- 偽 TNF- 偽 Th17 Treg. From October 2015 to August 2016, adolescents with acne were admitted to the Department of Dermatology in Shan County Central Hospital.
