【摘要】：研究背景： 斑禿是臨床皮膚科中常見(jiàn)的疾病,治療方式多種多樣,紫外線治療也是其中的一種方法。308nm準(zhǔn)分子光是新近適用于臨床的中波紫外線光,其治療斑禿的作用機(jī)理與誘導(dǎo)皮損T細(xì)胞凋亡、抑制細(xì)胞因子產(chǎn)生和影響抗原提呈細(xì)胞功能等有關(guān)[1]。中醫(yī)中藥治療斑禿有特色,根據(jù)辨證論治均能取得較滿(mǎn)意的療效。 研究目的： 觀察神應(yīng)養(yǎng)真丹聯(lián)合308nm準(zhǔn)分子光治療血虛風(fēng)燥型斑禿的臨床療效。 研究方法： 1.研究對(duì)象：選擇2011年3月至2012年2月南京中醫(yī)藥大學(xué)第一附屬醫(yī)院江蘇省中醫(yī)院皮膚科門(mén)診確診的斑禿患者45例。 2.研究方法：將45例斑禿患者隨機(jī)分為3組。神應(yīng)養(yǎng)真丹+308nm準(zhǔn)分子光組即Ⅰ組(15例共22塊皮損)予口服神應(yīng)養(yǎng)真丹,每次5g,每日2次,并聯(lián)合308nm準(zhǔn)分子光治療,每周2次；308nm準(zhǔn)分子光組即Ⅱ組(15例共23塊皮損)予308nm準(zhǔn)分子光治療,每周2次；神應(yīng)養(yǎng)真丹組即Ⅲ組(15例共15塊皮損)予口服神應(yīng)養(yǎng)真丹,每次5g,每日2次。3個(gè)治療組觀察療程均為12周,在治療前后分別對(duì)觀察的靶皮損進(jìn)行拍照,同時(shí)觀察靶皮損的局部不良反應(yīng)。 研究結(jié)果： 1.神應(yīng)養(yǎng)真丹+308nm準(zhǔn)分子光組共15例患者,其中男性6例,女性9例,平均年齡43.40歲,平均病程11.67月；308nm準(zhǔn)分子光組共15例患者,其中男性6例,女性9例,平均年齡38.73歲,平均病程11.47月；神應(yīng)養(yǎng)真丹組共15例患者,其中男性7例,女性8例,平均年齡36.47歲,平均病程12.67月。 2.神應(yīng)養(yǎng)真丹+308nm準(zhǔn)分子光組15例患者共22片皮損中痊愈13片,顯效5片,有效2片,無(wú)效2片,總有效率81.8%;308nm準(zhǔn)分子光組15例患者共23片皮損中痊愈6片,顯效6片,有效7片,無(wú)效4片,總有效率52.2%；神應(yīng)養(yǎng)真丹組15例患者共15塊皮損中痊愈1片,顯效2片,有效6塊,無(wú)效6片,總有效率20.0%。 3.治療中靶皮損未見(jiàn)明顯不良反應(yīng)。 研究結(jié)論： 1.聯(lián)合組的靶皮損毛發(fā)增長(zhǎng)率高于單獨(dú)使用308nm準(zhǔn)分了光組或單獨(dú)口服神應(yīng)養(yǎng)真丹組,308nm準(zhǔn)分子光組的靶皮損毛發(fā)增長(zhǎng)率大于神應(yīng)養(yǎng)真丹組； 2.三組患者治療后靶皮損療效評(píng)分明顯降低,治療前后積分差異存在統(tǒng)計(jì)學(xué)差異(p0.05)；3.三組患者靶皮損不同分期總有效率比較不存在統(tǒng)計(jì)學(xué)差異(p0.05)； 4.聯(lián)合組的靶皮損總有效率高于單獨(dú)使用308nm準(zhǔn)分子光組或單獨(dú)口服神應(yīng)養(yǎng)真丹組,308nm準(zhǔn)分子光組的總有效率大于神應(yīng)養(yǎng)真丹組； 5.對(duì)單獨(dú)和聯(lián)合組使用308nm準(zhǔn)分了光組光療的總劑量進(jìn)行比較,兩組照光總劑量比較,P0.05,均有統(tǒng)計(jì)學(xué)差異；
[Abstract]:Background: Alopecia areata is a common disease in clinical dermatology, and there are many ways to treat it. The therapeutic mechanism of alopecia areata is related to inducing T cell apoptosis, inhibiting cytokine production and affecting the function of antigen presenting cells. Traditional Chinese medicine treatment of alopecia areata has characteristics, according to syndrome differentiation can achieve satisfactory results. Objectives of the study: To observe the clinical effect of Shenying Yangzhen Dan combined with 308nm excimer light in treating alopecia areata with blood deficiency and wind dryness. Research methods: 1. Participants: from March 2011 to February 2012, 45 patients with alopecia areata were selected from Department of Dermatology, Jiangsu traditional Chinese Medicine Hospital, first affiliated Hospital of Nanjing University of traditional Chinese Medicine. 2.
