【摘要】： 目的：中藥內(nèi)服、中藥外敷、光子嫩膚技術(shù)三者結(jié)合(以下稱綜合治療組)治療黃褐斑,并通過(guò)與中藥治療、光子治療的單獨(dú)應(yīng)用做對(duì)照比較,觀察色斑面積及顏色變化,采用評(píng)分法研究他們治療黃褐斑療效的差別,篩選治療黃褐斑的優(yōu)化治療方案,以提高黃褐斑的臨床治療有效率。 實(shí)施方法： 1、采用臨床隨機(jī)同期對(duì)照試驗(yàn),預(yù)先將150例虛擬病例編成1,2,3,……150號(hào),以序號(hào)為變量名輸入DPS統(tǒng)計(jì)軟件進(jìn)行隨機(jī)分組,以患者就診順序?yàn)樾蛱?hào),電腦產(chǎn)生的隨機(jī)數(shù)字決定歸組,執(zhí)行時(shí)實(shí)施分配隱匿。自接受治療之日起,每四周進(jìn)行一次療效評(píng)估。 2、選取符合臨床診斷標(biāo)準(zhǔn)及納入觀察標(biāo)準(zhǔn)的黃褐斑患者150例,按電腦產(chǎn)生的隨機(jī)數(shù)字分為三組：中藥治療組、光子治療組、綜合治療組,每組50例。 3、中藥治療組：口服百花養(yǎng)顏蜜,每次一袋(6g),一日三次,飯前服,服藥期間忌生冷、酒,規(guī)律飲食、忌熬夜,一月為一療程,連續(xù)三個(gè)療程,同時(shí)面部外敷百花養(yǎng)顏1號(hào),每日一次,每次三十分鐘,治療期間停用其他帶有特殊療效護(hù)膚品及藥物,一個(gè)月為一個(gè)療程,連續(xù)三個(gè)療程；光子治療組：接受十堰市中醫(yī)院中醫(yī)美容�？崎T診光子治療,每月一次,連續(xù)三次,治療期間停用其他帶有特殊療效護(hù)的護(hù)膚品及藥物,防曬。綜合治療組：接受綜合治療：首先做光子治療,術(shù)后立即外敷中藥,即日起內(nèi)服中藥,內(nèi)服與外敷療法同中藥治療組。 4、建立實(shí)驗(yàn)觀察病歷,詳細(xì)記錄患者相關(guān)資料。采用評(píng)分法觀察記錄3組治療前、治療后色斑面積及顏色的變化,實(shí)驗(yàn)病例數(shù)達(dá)130例時(shí)進(jìn)行組間均衡行檢測(cè),根據(jù)結(jié)果進(jìn)行調(diào)整。 5、匯總觀察數(shù)據(jù),進(jìn)行統(tǒng)計(jì)學(xué)分析：根據(jù)課題所需回答的問題(比較三種療法的療效差異性)、資料的類型(有序分類資料)以及實(shí)驗(yàn)設(shè)計(jì)的方法(隨機(jī)同期對(duì)照試驗(yàn)),三組療法療效比較的統(tǒng)計(jì)分析方法選擇RxC表資料分析,Ridit分析,統(tǒng)計(jì)軟件為SPSS17.0.;三組患者的一般資料做方差齊性檢驗(yàn)。 結(jié)果：三組療效比較經(jīng)Ridit分析結(jié)論：本例三組R均值95%CI無(wú)重疊,按a=0.05水準(zhǔn)認(rèn)為差別有統(tǒng)計(jì)學(xué)意義。其中： 光子組：R均值=0.6753, CI為(0.6091,0.7443) 中藥組：R均值=0.5481, CI為(0.4709,0.6254) 綜合組：R均值=0.3996, CI為(0.3393,0.4539) 療效等級(jí)是由“好”到“差”的順序排列的。R均值較小者療效好,故綜合組治療黃褐斑的療效優(yōu)于其他兩組。 結(jié)論：中藥內(nèi)服,中藥外敷,光子兩組嫩膚技術(shù)三者結(jié)合的綜合治療組療效高,不良反應(yīng)少,優(yōu)于單用中藥組和單用光子組。
[Abstract]:Objective: to treat chloasma with the combination of traditional Chinese medicine (TCM), external application of Chinese medicine (TCM) and photon tender skin technique (hereinafter referred to as the comprehensive treatment group), and to observe the area and color change of the spot by comparing with the single application of traditional Chinese medicine and photon therapy.In order to improve the effective rate of clinical treatment of chloasma, the difference of their curative effect was studied by using the scoring method, and the optimized treatment scheme was screened out in order to improve the clinical treatment rate of chloasma.Methods of implementation:1. In a randomized controlled trial, 150 virtual cases were prearranged into 1 / 2 / 3 / 150. Conclusion: there was no overlap of 95%CI in the three groups, and the difference was statistically significant according to A0. 05 level.Of which:The mean value of the photonic group is 0.6753, CI is 0.6091 / 0.7443)The mean value was 0.5481, CI was 0.4709 / 0.6254 in Chinese herbal medicine group (n = 0. 548 1, CI = 0. 4709)The mean value of 0: r is 0.3996, CI is 0.3393, 0.4539)The rank of curative effect is arranged from "good" to "bad". The mean value of .R is better than that of the other two groups, so the curative effect of the comprehensive group is better than that of the other two groups in the treatment of chloasma.Conclusion: the comprehensive treatment group with combination of traditional Chinese medicine, external application of traditional Chinese medicine and photon tonifying technique has higher curative effect and fewer adverse reactions, which is superior to the traditional Chinese medicine group and the single photon group.
