【摘要】：目的探討咽喉部梅毒的鑒別和診斷要點。方法回顧性分析2011年2月-2015年1月在本院耳鼻咽喉科以咽喉癥狀及咽喉部黏膜損害為特征首診的21例Ⅱ期梅毒患者的臨床資料,包括纖維喉鏡檢查結(jié)果、皮損部位和形態(tài)及臨床特征等。結(jié)果該21例患者主要病損區(qū)域均是以扁桃體和軟腭為中心的口咽部,累及舌根者8例,伴喉部黏膜受損者5例。病損黏膜均表現(xiàn)為周圍伴有紅色充血帶的黏膜白斑,未發(fā)現(xiàn)單獨存在黏膜紅斑的病例。喉部黏膜均伴有非特異性炎癥,其中4例聲嘶患者聲帶充血和腫脹,黏膜呈白斑樣增生�？姑范局委煹念A后均較好。結(jié)論咽喉部Ⅱ期梅毒病損區(qū)域多為扁桃體和軟腭為中心的口咽部,但常伴有喉炎,并可加重聲帶的機械性損傷。病損主要為黏膜白斑,白斑周圍伴潮紅色充血帶,結(jié)合病損局部檢查(病理活檢和滲出物涂片)和梅毒血清實驗室檢查可以確診,青霉素為該病治療的一線用藥。
[Abstract]:Objective to study the differential diagnosis and diagnosis of pharynx and larynx syphilis.Site, morphology and clinical features of skin lesions.Results the main lesion areas of 21 patients were the oropharynx with tonsil and soft palate as the center, including 8 cases with tongue root and 5 cases with laryngeal mucosal damage.Leukoplakia with red hyperemia was found in all the lesions, and there was no single case of erythema.Non-specific inflammation was found in all laryngeal mucosa, including hyperemia and swelling of vocal cord in 4 patients with hoarseness and leukoplakia.The prognosis of anti-syphilis treatment was better.Conclusion the pharyngeal and laryngeal secondary syphilis lesions are mostly tonsil and soft palate centered oropharynx, but often accompanied by laryngitis, and may aggravate the mechanical injury of vocal cord.The lesions were mainly mucosal leukoplakia with damp red hyperemia around the leukoplakia combined with local examination (pathological biopsy and exudate smear) and syphilis serum laboratory examination. Penicillin was the first-line drug for the treatment of the disease.
