【摘要】：目的:采用視覺模擬量表、連續(xù)7天蕁麻疹活動度量表及慢性蕁麻疹生活質(zhì)量量表對患者進行調(diào)查,探討慢性自發(fā)性蕁麻疹患者血清中IL-31表達水平與蕁麻疹嚴重程度、瘙癢及生活質(zhì)量的相關(guān)性。方法:收集2016年6月~2016年12月,就診于山西醫(yī)科大學第一醫(yī)院皮膚科并確診為慢性自發(fā)性蕁麻疹患者61例和同期隨機在我院健康體檢者59例的臨床一般資料及外周血標本,所有患者均符合慢性自發(fā)性蕁麻疹診斷標準。采用ELISA法檢測所有標本中血清IL-31的含量,通過瘙癢持續(xù)時間、視覺模擬量表、連續(xù)7天蕁麻疹活動度量表及慢性蕁麻疹生活質(zhì)量量表等獲取臨床資料。采用SPSS22.0軟件對數(shù)據(jù)進行處理,得到患者血清中IL-31與瘙癢、蕁麻疹嚴重程度和生活質(zhì)量的相關(guān)性。結(jié)果:對照組IL-31水平為(1.74±0.30pg/ml),研究組IL-31水平為(28.63±15.38pg/ml),兩組相比差異具有統(tǒng)計學意義(Z=-9.095,P0.001)。CSU患者輕度、中度及重度瘙癢組之間IL-31水平差異有統(tǒng)計學意義(P0.05)。CSU患者瘙癢持續(xù)時間30min組、瘙癢持續(xù)時間30min-120min組、瘙癢持續(xù)時間120min組之間IL-31水平差異沒有統(tǒng)計學意義(P=0.238);疾病嚴重程度與CSU患者IL-31水平呈正相關(guān)(r值為0.665,P0.001)。患者生活質(zhì)量與IL-31水平呈正相關(guān)(r值為0.569,P0.001)。結(jié)論:IL-31在CSU患者血清中的表達升高,且與瘙癢程度、疾病嚴重程度及生活質(zhì)量呈正相關(guān),而與瘙癢持續(xù)時間無相關(guān)性,表明IL-31可能參與CSU的發(fā)病,并可能與其瘙癢有關(guān)。IL-31有可能成為評估CSU嚴重程度及對生活質(zhì)量影響的指標,為臨床診療提供指導。
[Abstract]:Objective: to investigate the expression of IL-31 and the severity of urticaria in patients with chronic urticaria by using visual analogue scale, activity scale for 7 days and quality of life scale for chronic urticaria. The correlation between itching and quality of life. Methods: from June 2016 to December 2016, Clinical general data and peripheral blood samples of 61 patients with chronic spontaneous urticaria diagnosed in the Department of Dermatology, first Hospital of Shanxi Medical University and 59 cases of random physical examination in our hospital at the same time. All patients met the diagnostic criteria for chronic spontaneous urticaria. Serum IL-31 levels were measured by ELISA method. The clinical data were obtained by urticaria activity scale and chronic urticaria quality of life scale for 7 days. The data were processed by SPSS22.0 software and the IL-31 and pruritus in serum were obtained. Results: the level of IL-31 in the control group was 1.74 鹵0.30pg / ml, and the IL-31 level in the study group was 28.63 鹵15.38pg / ml. The difference between the two groups was statistically significant. There were significant differences in IL-31 levels between moderate and severe pruritus groups. There were significant differences between 30min group and 30min-120min group. There was no significant difference in IL-31 level between 120min group and CSU group, and there was a positive correlation between the severity of the disease and IL-31 level in CSU patients. There was a positive correlation between quality of life (QOL) and IL-31 level in patients with CSU. Conclusion IL-V31 is positively correlated with the level of IL-31 in patients with CSU. The expression in Qing Dynasty was increased, There was a positive correlation with the degree of pruritus, severity of disease and quality of life, but no correlation with the duration of pruritus, indicating that IL-31 may be involved in the pathogenesis of CSU. It may be related to itching. IL-31 may be an index to evaluate the severity of CSU and its impact on quality of life, and provide guidance for clinical diagnosis and treatment.
