麻風(fēng)皮損中Th17細(xì)胞及RORγt的表達(dá)及意義

【摘要】:背景:麻風(fēng)病是由麻風(fēng)分枝桿菌引起皮膚和外周神經(jīng)受累的慢性傳染病,是導(dǎo)致人類致殘的主要疾病之一。人類是麻風(fēng)分枝菌的主要宿主和傳染源,未經(jīng)治療的麻風(fēng)病人(主要是多菌型患者)可以通過呼吸道和皮膚進(jìn)行傳染。但有研究表明,在與傳染性麻風(fēng)患者同等接觸的條件下,只有少數(shù)人發(fā)病,這是因?yàn)槁轱L(fēng)患者存在著不同程度的細(xì)胞免疫缺陷,但其細(xì)胞免疫缺陷的確切機(jī)制尚不明了。近來Th17細(xì)胞逐漸被認(rèn)為是一個(gè)獨(dú)立的輔助T細(xì)胞亞群,具有抑制其他T細(xì)胞活化的功能,即具有免疫調(diào)節(jié)或具有抑制作用。孤獨(dú)核受體(Orphan nuclear receptor,RORyt)是控制Th17細(xì)胞分化的轉(zhuǎn)錄因子。有研究報(bào)道,結(jié)核病的發(fā)病與Th17細(xì)胞及RORyt有關(guān)。麻風(fēng)病與結(jié)核病有許多相似之處。因此,本文主要研究Thl7細(xì)胞及RORyt與麻風(fēng)病發(fā)病的關(guān)系。目的:研究麻風(fēng)患者皮損中Th17細(xì)胞及RORyt的表達(dá),闡明Th17細(xì)胞及RORyt與麻風(fēng)發(fā)病的關(guān)系。 方法:1、麻風(fēng)患者組:選擇30例麻風(fēng)患者,根據(jù)國際標(biāo)準(zhǔn)將患者分為TT、BT、 BB、BL及LL型,用免疫組化測皮損中Th17細(xì)胞及RORγt的分布。 2、健康對照組:為門診健康體檢者20例,用免疫組化測皮損中Th17細(xì)胞及RORyt的分布。 結(jié)果:1、Th17細(xì)胞主要位于麻風(fēng)皮損真皮層,并表達(dá)于胞漿;正常對照皮膚中Th17細(xì)胞數(shù)量減少或無。BT型和TT型中Thl7細(xì)胞分布在肉芽腫及其周圍。LL型和BL型Th17細(xì)胞分布在侵潤的巨噬細(xì)胞間。本實(shí)驗(yàn)結(jié)果顯示,Th17細(xì)胞分布與巨噬細(xì)胞有關(guān),提示Th17細(xì)胞和組織細(xì)胞之間可能存在相互作用。 2、RORyt主要位于麻風(fēng)皮損的真皮層,且表達(dá)于胞漿;正常對照皮膚中RORyt數(shù)量減少或無。BT型和TT型中RORyt分布在肉芽腫及其周圍。LL型和BL型中RORyt分布在侵潤的巨噬細(xì)胞間。 結(jié)論:1、Th17細(xì)胞是麻風(fēng)病的重要參與者2、提示Th17細(xì)胞與組織細(xì)胞之間存在相互作用。3、Th17細(xì)胞在LL、BL、BT、TT中呈逐漸增高的趨勢,提示Th17細(xì)胞可能對麻風(fēng)患者有保護(hù)作用。4、實(shí)驗(yàn)組RORyt高于健康對照組,提示麻風(fēng)患者發(fā)病可能與RORyt相關(guān),并且通過調(diào)節(jié)Th17細(xì)胞的分化而發(fā)生作用。RORyt表達(dá)在LL、BL、BT、TT中呈逐漸增高的趨勢,提示RORyt可能對麻風(fēng)患者有保護(hù)作用。5、Th17細(xì)胞具有抵抗細(xì)胞外感染的作用,能在局部引起炎癥,抵抗外來病原體。
[Abstract]:Background: leprosy is a chronic infectious disease of skin and peripheral nerve caused by Mycobacterium leprosy. It is one of the major diseases causing human disability. Human is the main host and source of infection of Mycobacterium leprosy. Untreated leprosy patients (mainly multicellular) can be transmitted through the respiratory tract and skin. But studies have shown that only a small number of people develop disease under the same conditions as people with infectious leprosy. This is due to varying degrees of cellular immune deficiency in leprosy patients, but the exact mechanism of cellular immune deficiency remains unclear. Recently, Th17 cells have been recognized as an independent helper T cell subgroup. It has the function of inhibiting the activation of other T cells, that is, it has immunomodulation or inhibitory effect. Orphan nuclear receptor RORytis a transcription factor controlling the differentiation of Th17 cells. The pathogenesis of tuberculosis is related to Th17 cells and RORyt. There are many similarities between leprosy and leprosy. Therefore, the relationship between Thl7 cells and RORyt and leprosy was studied. Objective: to study the expression of Th17 cells and RORyt in leprosy skin lesions. To elucidate the relationship between Th17 cells and RORyt and leprosy. Methods Twenty patients with leprosy were divided into BTT, BBBBL and LL according to international standards. The distribution of Th17 cells and ROR 緯 t in lesions were detected by immunohistochemistry. 2. The distribution of Th17 cells and RORyt in skin lesions were detected by immunohistochemistry. Results the Th17 cells were mainly located in the dermis of leprosy and expressed in the cytoplasm. The number of Th17 cells in normal control skin decreased or did not. BT type and TT type Thl7 cells distributed in granuloma and its surrounding. LL and BL types of Th17 cells distributed in infiltrated macrophages. The results showed that the distribution of Th 17 cells was related to macrophages. These results suggest that there may be interaction between Th17 cells and histocytes. 2RORyt was mainly located in the dermis of leprosy and expressed in cytoplasm, and RORyt in normal skin decreased or did not exist in BT type and TT type in granuloma and its surrounding. LL and BL types, RORyt was distributed between invading macrophages. Conclusion Th17 cells are important participants in leprosy, suggesting that the interaction between Th17 cells and histiocytes. The results suggest that Th17 cells may have protective effect on leprosy patients. The RORyt of experimental group is higher than that of healthy control group, suggesting that the pathogenesis of leprosy patients may be related to RORyt, and the expression of .RORyt may increase gradually by regulating the differentiation of Th17 cells. The results suggest that RORyt may have protective effect on leprosy patients. 5. Th17 cells can resist extracellular infection, and can cause inflammation locally and resist foreign pathogens.
