【摘要】：目的:研究斑禿患者的臨床特征,分析影響斑禿病情輕重的相關(guān)因素,為臨床診治和預(yù)后判定提供依據(jù)。 方法:收集我院皮膚科門診就診的314例斑禿患者的臨床資料,對性別、年齡、初發(fā)病年齡、病程、復(fù)發(fā)、脫發(fā)面積、伴隨個人和/或及家族中過敏性疾病史、自身免疫性疾病史、家族史和甲病變等臨床特征進行統(tǒng)計分析。計量資料采用t檢驗,計數(shù)資料比較采用χ2檢驗,并進行多因素Logistic回歸分析。 結(jié)果:314例斑禿患者中,男性160例(50.9%),女性154例(49.1%),男女性別比為1.04:1。初發(fā)病年齡為1～74歲,平均初發(fā)病年齡為(28.48±14.05)歲。病程為1天～18年,中位病程為4月。輕型AA (輕型組) 198例(63.1%),重型AA (重型組) 116例(36.9%)。10.1%的斑禿患者家族史陽性,36.9%的斑禿患者伴隨甲病變,15.6%的斑禿患者有個人和/或家族性過敏性疾病史,5.4%斑禿患者有自身免疫性疾病史。重型組的病程顯著長于輕型組(t=-7.337, P 0. 05),初發(fā)病年齡顯著小于輕型組(t=2.910, P 0. 05),伴隨有甲病變所占的比例顯著高于輕型組(χ2 =28.784, P 0. 05),輕型組與重型組之間性別、復(fù)發(fā)、伴隨個人和/或家族性過敏性疾病史、自身免疫性疾病史和家族史所占比例比較,差異均無統(tǒng)計學(xué)意義(P均0.05)。經(jīng)多因素Logistic回歸分析,病程、初發(fā)病年齡和伴隨甲病變與斑禿病情嚴重程度有關(guān)。 結(jié)論:病程、初發(fā)病年齡和伴隨甲病變與斑禿病情嚴重程度有關(guān)。病程長、初發(fā)病年齡早和伴隨甲病變是重型斑禿的危險因素。斑禿病情的嚴重程度與性別、復(fù)發(fā)、家族史、伴有個人和/或家族中過敏性疾病史和自身免疫性疾病史無關(guān)。
[Abstract]:Objective: to study the clinical features of patients with alopecia areata and analyze the related factors affecting the severity of alopecia areata. Methods: the clinical data of 314 patients with alopecia areata were collected, including sex, age, age of onset, course of disease, recurrence, area of alopecia alopecia, history of allergic diseases in individuals and / or families. The clinical features of autoimmune disease history, family history and nail lesion were statistically analyzed. The course of disease was analyzed by multivariate Logistic regression analysis. The age of onset and associated nail lesions were associated with the severity of alopecia areata. Conclusion: the severity of alopecia areata is related to the course of disease, the age of initial onset and the concomitant nail lesion. The long course of disease, the early age of onset and accompanying nail lesion are the risk factors of severe alopecia areata, the severity and sex of alopecia areata disease, recurrence, family history, etc. There is no correlation between the history of allergic diseases and autoimmune diseases in individuals and / or families.
