【摘要】：目的觀察硬膜外神經阻滯聯(lián)合脊神經背根脈沖射頻治療老年帶狀皰疹后遺神經痛的臨床療效。方法將70例帶狀皰疹后遺神經痛老年患者隨機分為觀察組和對照組各35例,兩組均口服加巴噴丁并行硬膜外神經阻滯,觀察組加用脊神經背根脈沖射頻。治療前與治療后4周,采用視覺模擬評分法(VAS)評價疼痛程度,分別采用焦慮自評量表(SAS)、抑郁自評量表(SDS)評價焦慮、抑郁情況,評價兩組臨床療效。結果治療前兩組VAS及SDS、SAS評分比較,差異無統(tǒng)計學意義(P均0.05);治療后兩組VAS及SDS、SAS評分均低于治療前,且觀察組VAS及SDS、SAS評分低于對照組,差異有統(tǒng)計學意義(P均0.05)。觀察組治療總有效率94.29%,低于對照組的77.14%(P0.05)。結論采用硬膜外神經阻滯聯(lián)合脊神經背根脈沖射頻輔助治療帶狀皰疹后遺神經痛,可減輕患者疼痛,間接改善其不良情緒,有助于提高臨床療效。
[Abstract]:Objective to observe the clinical effect of epidural nerve block combined with pulse radiofrequency of dorsal root of spinal nerve in the treatment of postherpetic neuralgia in elderly patients. Methods 70 cases of senile patients with postherpetic neuralgia were randomly divided into observation group (n = 35) and control group (n = 35). Both groups were given gabapentin and epidural nerve block, and the observation group were treated with spinal dorsal root pulse radiofrequency. Visual analogue score was used to evaluate the degree of pain before and 4 weeks after treatment. Self rating anxiety scale (SAS) and self rating Depression scale (SDS) were used to evaluate anxiety and depression, and to evaluate the clinical efficacy of the two groups. Results before treatment, the scores of VAS and SAS were compared between the two groups. After treatment, the scores of VAS and SAS in the two groups were lower than those before treatment, and the scores of VAS and SAS in the observation group were lower than those in the control group. The total effective rate of treatment in the observation group was 94.29, which was lower than that in the control group (77.14%, P 0.05). Conclusion the treatment of postherpetic neuralgia with epidural nerve block combined with pulse radiofrequency of dorsal root of spinal nerve can alleviate the pain of the patients. Indirectly ameliorating its bad emotion is helpful to improve the clinical curative effect.
