老化皮膚光學(xué)特征提取及其治療過程監(jiān)測

【摘要】:皮膚的老化分為兩個(gè)過程,即:隨年齡增加的自然老化和受外界紫外光照射引起的光老化。這兩個(gè)老化過程總是相互交叉,相互影響的。膠原的變化是皮膚老化過程中最主要的表征量。本論文結(jié)合多光子顯微技術(shù)、光學(xué)相干層析成像技術(shù)等先進(jìn)的醫(yī)學(xué)成像技術(shù),從組織光學(xué)、形態(tài)學(xué)、光譜及紋理特征等方面研究活體皮膚自然老化和光老化的過程,并將結(jié)果用于評(píng)價(jià)皮膚的光治療過程。 具體的研究創(chuàng)新成果如下: 1.利用多光子顯微技術(shù)和光學(xué)相干層析成像技術(shù)活體跟蹤研究自然老化及光老化過程中皮膚膠原形態(tài)及光譜方面的變化。結(jié)果顯示真皮的膠原尺寸不隨著年齡的變化而變化,而膠原二次諧波強(qiáng)度及最大可探測深度隨著年齡的變化而其較大變化。 2.把膠原紋理特征參數(shù)作為評(píng)價(jià)各種老化類型皮膚的新指標(biāo)。具體的有膠原方向指數(shù)、膠原束間距離、膠原相關(guān)性、膠原熵、分形維數(shù)及光學(xué)特性參數(shù)等。結(jié)果顯示:年輕皮膚的膠原方向指數(shù)小,趨向各向同性,而且膠原束間距小,紋理緊密,細(xì)紋多。而自然老化皮膚的膠原方向指數(shù)大,膠原間距大;光老化皮膚的膠原在紋理上沒有特別的方向性,但是膠原間距很大。 3.把光學(xué)特性參數(shù)作為評(píng)價(jià)皮膚老化過程的新指標(biāo),通過光學(xué)相干層析成像技術(shù)獲得小鼠皮膚不同層的衰減系數(shù)和皮膚表皮厚度隨著年齡和紫外光輻照程度變化的曲線圖:皮膚真皮層的衰減系數(shù)隨著年齡的增大而減小,也受紫外光的影響。 4.結(jié)合多光子顯微技術(shù)與光學(xué)相干層析成像技術(shù)等醫(yī)學(xué)成像技術(shù)從形態(tài)學(xué)、紋理特征、組織光學(xué)的角度探討各種光源輻照皮膚后,皮膚真皮膠原及表皮的反應(yīng),并跟蹤隨后的修復(fù)過程。從亞微米數(shù)量級(jí)到毫米數(shù)量級(jí)來獲得光對(duì)各種類型皮膚的光治療情況。提出治療的關(guān)鍵點(diǎn),并評(píng)價(jià)各種光源對(duì)不同類型皮膚治療的效果:消融性的IPL, Nd:YAG只能用于改善細(xì)小的皺紋,而對(duì)于嚴(yán)重的光老化皮膚則沒有明顯的效果。消融性的Er:YAG, C02激光對(duì)皮膚作用強(qiáng),會(huì)引起較嚴(yán)重的并發(fā)癥,但是可以較好的應(yīng)用于光老化皮膚的治療。其結(jié)果對(duì)光美容臨床應(yīng)用具有較大的指導(dǎo)意義。
[Abstract]:The aging of the skin is divided into two processes: natural aging with age and photoaging caused by external ultraviolet light. The two aging processes always intersect each other. The changes of collagen are the most important characterizations of skin aging. In this paper, advanced medical imaging techniques, such as multiphoton microscopy, optical coherence tomography, etc., have been developed from tissue optics, morphology, histopathology, histopathology, histopathology, histopathology, and other advanced medical imaging techniques. The natural aging and photoaging of living skin were studied in terms of spectral and texture characteristics, and the results were used to evaluate the process of skin phototherapy. Specific research and innovation results are as follows:. 1. The changes of collagen morphology and spectrum during natural aging and photoaging were studied in vivo by using multiphoton microscopy and optical coherence tomography. The results showed that the collagen size of dermis did not change with age. The second harmonic intensity and maximum detectable depth of collagen vary greatly with age. 2. The characteristic parameters of collagen texture were taken as a new index to evaluate various aging skin types, including collagen orientation index, distance between collagen bundles, collagen correlation, collagen entropy, collagen orientation index, collagen correlation, collagen entropy, collagen correlation and collagen entropy. The results show that the collagen direction index of young skin is small, tends to be isotropic, and the distance between collagen bundles is small, the texture is tight, the fine grain is many, but the collagen direction index of natural aging skin is large. Collagen spacing is large; the collagen of photoaged skin is not particularly directional in texture, but the collagen spacing is very large. 3. Using the optical characteristic parameters as a new index to evaluate the aging process of skin, The attenuation coefficient of different layers of mouse skin and the curve of skin skin thickness with age and ultraviolet irradiation degree were obtained by optical coherence tomography. The attenuation coefficient of dermis decreased with age. Also affected by ultraviolet light. 4. Combining medical imaging techniques such as multi-photon microscopy and optical coherence tomography, the reaction of dermal collagen and epidermis to skin irradiated by various light sources was discussed from the point of view of morphology, texture and tissue optics. And follow up the subsequent repair process. From submicron to millimeter order of magnitude to get light for various types of skin phototherapy. Put forward the key points of treatment, The effects of various light sources on the treatment of different skin types were evaluated. The ablative IPL, Nd:YAG can only be used to improve the fine wrinkles, but not for the severe photoaged skin. The ablative er: YG, C02 laser has a strong effect on the skin. It can cause serious complications, but it can be applied to the treatment of photoaged skin. The results are of great guiding significance for the clinical application of photo-cosmetology.
