【摘要】：目的研究天皰瘡患者血清中抗橋粒芯蛋白(desmoglein,Dsg)1和抗Dsg3抗體水平在天皰瘡的臨床診斷價值。方法采用酶聯(lián)免疫吸附試驗(ELISA)測定62例天皰瘡患者血清中抗Dsg1和抗Dsg3抗體水平;間接免疫熒光法(IIF)測定天皰瘡抗體Ig G水平。結(jié)果 62例天皰瘡患者中抗Dsg1抗體或抗Dsg3抗體任一陽性共50例,陽性率80.65%,與間接免疫熒光法(IIF)相比兩種方法差異無統(tǒng)計學意義(P0.05)。因此抗Dsg抗體具有和IIF一樣的臨床診斷價值。34例尋常型天皰瘡(PV)中抗Dsg3抗體陽性或抗Dsg1和抗Dsg3抗體均陽的28例,陽性率82.35%;24例落葉型天皰瘡(PF)中抗Dsg1抗體陽性的為20例,陽性率83.33%�？梢娍笵sg3抗體是PV的鑒別診斷指標,抗Dsg1抗體是PF的鑒別診斷指標。此外,通過對治療前后抗Dsg1抗和Dsg3抗體的滴度水平的檢測發(fā)現(xiàn),抗Dsg1和抗Dsg3抗體還可作為天皰瘡治療監(jiān)測的指標。結(jié)論采用ELISA方法測定天皰瘡患者外周血抗Dsg1抗體和抗Dsg3抗體,方法簡便,對患者創(chuàng)傷小,有一定的診斷和鑒別診斷價值。另外,抗Dsg1抗體和抗Dsg3抗體還可作為監(jiān)測天皰瘡治療效果的指標。
[Abstract]:Objective to study the antidesmoglein in serum of pemphigus patients. Clinical diagnostic value of Dsg)1 and anti Dsg3 antibody levels in pemphigus. Methods Elisa was used to detect pemphigus. The levels of anti Dsg1 and anti Dsg3 antibodies were measured in 62 patients with pemphigus. Results in 62 patients with pemphigus, 50 were positive for either anti-pemphigus antibody or anti-pemphigus antibody. The positive rate was 80.65%. There was no significant difference between the two methods in comparison with the indirect immunofluorescence assay (IIF). Therefore, the anti Dsg antibody has the same clinical value as IIF in the diagnosis of pemphigus vulgaris. There were 28 cases of positive or anti-Dsg1 and anti-Dsg3 antibodies. The positive rate was 82.35. Among 24 cases of deciduous pemphigus pemphigus, 20 cases were positive for anti-pemphigus antibody, the positive rate was 83.33. It can be seen that anti-pemphigus antibody is the differential diagnostic index of PV. Anti Dsg1 antibody is the differential diagnostic index of PF. In addition, the titers of anti Dsg1 antibody and Dsg3 antibody before and after treatment were detected. Anti Dsg1 and anti Dsg3 antibodies can also be used as indicators for monitoring the treatment of pemphigus. Conclusion the ELISA method for detection of anti Dsg1 antibody and anti Dsg3 antibody in peripheral blood of pemphigus patients is simple and convenient. In addition, anti Dsg1 antibody and anti Dsg3 antibody can also be used as indicators for monitoring the therapeutic effect of pemphigus.
