高遷移率族蛋白B1在糖尿病腎病中的表達(dá)及意義

【摘要】:目的:檢測糖尿病腎病患者血清中高遷移率族蛋白B1(HMGB1)及相關(guān)炎癥因子和氧化應(yīng)激指標(biāo),探討HMGB1在糖尿病腎病發(fā)生發(fā)展中的作用。方法:收集120例臨床糖尿病腎病及60例單純糖尿病患者的臨床資料及血液標(biāo)本。臨床糖尿病腎病患者按24 h尿白蛋白定量分為微量白蛋白尿組(n=60)和大量白蛋白尿組(n=60)。檢測血液中糖化血紅蛋白(Hb A1c)、總膽固醇(TC)、低密度脂蛋白膽固醇(LDL-C)、高密度脂蛋白膽固醇(HDL-C)、甘油三酯(TG)、肌酐等水平,并采用酶聯(lián)免疫吸附(ELISA)法檢測血清中HMGB1、晚期糖基化終末產(chǎn)物受體(RAGE)、丙二醛(MDA)、腫瘤壞死因子α(TNF-α)濃度。變量之間相關(guān)性分析采用Pearson相關(guān)性分析。結(jié)果:1臨床糖尿病腎病組血清中HMGB1、RAGE、TNF-α、MDA水平高于單純糖尿病組,且差異具有統(tǒng)計(jì)學(xué)意義(P0.01);2HMGB1、RAGE、TNF-α、MDA在大量白蛋白尿組中表達(dá)量最高;3HMGB1與RAGE、TNF-α、MDA呈正相關(guān)。結(jié)論 :HMGB1在臨床糖尿病腎病患者血清中水平顯著升高,且隨著蛋白尿加重,其血清中的水平亦升高。HMGB1可能通過促進(jìn)炎癥反應(yīng)及氧化應(yīng)激等機(jī)制參與糖尿病腎病發(fā)生發(fā)展。
