200例正常老年人性別及相關(guān)因素對(duì)腰椎骨密度的影響

【摘要】:目的探討200例不同性別正常老年人群腰椎骨密度(BMD)與性別因素及相關(guān)因素的相關(guān)性。方法雙能X線吸收法(DXA)BMD檢測(cè)儀檢測(cè)200例60歲及以上年齡的佳木斯地區(qū)常住人口中正常老年人群腰椎L2-4骨密度,記錄性別、年齡、體重、身高、BMI。化學(xué)發(fā)光酶免疫法檢測(cè)血清生長(zhǎng)激素(GH)、類胰島素樣生長(zhǎng)因子-1(IGF-1)、雌二醇(E2)、睪酮(T)。將所有人分為女性組和男性組,每組按5歲一個(gè)年齡段進(jìn)行分組,分為5組,60歲~,65歲~,70歲~,75歲~,≥80歲。分別進(jìn)行各變量與腰椎BMD及各變量之間的統(tǒng)計(jì)學(xué)分析。結(jié)果女性組中IGF-1、E2、GH、T與腰椎L2-4BMD有明顯正相關(guān),相關(guān)性IGF-1E2GHT;男性組中IGF-1、GH、E2、T與腰椎L2-4BMD有明顯正相關(guān),相關(guān)性IGF-1GHE2T;女性組、男性組年齡與腰椎L2-4BMD均有明顯負(fù)相關(guān)。所有組中性別因素與腰椎L2-4BMD有明顯相關(guān)。女性組控制年齡、身高、體重、BMI后,IGF-1與腰椎L2-4BMD有明顯正相關(guān)。男性組控制年齡、身高、體重、BMI后,IGF-1與腰椎L2-4BMD有明顯正相關(guān);E2與腰椎L2-4BMD有明顯正相關(guān),相關(guān)性IGF-1E2。所有組控制年齡、身高、體重、BMI后,性別因素與腰椎L2-4BMD有明顯相關(guān)。控制其他所有變量?jī)勺兞科嚓P(guān)及多元線性逐步回歸結(jié)果,顯示IGF-1與L2-4骨密度有明顯正相關(guān),性別與IGF-1有明顯負(fù)相關(guān),年齡與IGF-1有明顯負(fù)相關(guān),E2與IGF-1有明顯正相關(guān)。所有明顯相關(guān)性,P值均小于0.05。女性組和男性組身高、GH、IGF-1、E2、T隨年齡增長(zhǎng)均逐漸下降。結(jié)論性別因素對(duì)老年人群L2-4骨密度有影響,而性別因素的差異主要表現(xiàn)在女性組與男性組之間IGF-1有明顯差異,IGF-1與L2-4BMD明顯正相關(guān)。IGF-1隨年齡的下降是老年性骨質(zhì)疏松癥發(fā)生的重要原因,而雌激素能減慢IGF-1下降。老年男女性別差異對(duì)L2-4骨密度的影響與兩者IGF-1之間的差異有關(guān)。
