腦梗死合并亞臨床甲狀腺功能減退與內(nèi)脂素及內(nèi)皮素的關(guān)系

【摘要】:目的腦梗死患者中關(guān)于亞臨床甲狀腺功能和內(nèi)脂素、內(nèi)皮素關(guān)系的研究較少。文中旨在探討腦梗死合并亞臨床甲狀腺功能減退(簡稱甲減)患者甲狀腺功能與內(nèi)脂素、內(nèi)皮素的關(guān)系。方法入選2011年1月至2017年7月海南醫(yī)學院第二附屬醫(yī)院內(nèi)分泌科及老年病科腦梗死患者200例。將其分為4組:對照組[腦梗死合并甲狀腺功能正常,促甲狀腺激素(TSH)、游離甲狀腺素(FT4)、游離三碘甲狀腺原氨酸(FT3)均正常,n=40]、輕度亞臨床甲減組(4≤TSH10 m IU/L,FT3、FT4正常,n=60)、重度亞臨床甲減組(TSH≥10 m IU/L,FT3、FT4正常,n=60)、臨床甲減組(TSH≥4 m IU/L,FT3、FT4降低,n=40)。其中輕度亞臨床甲減組隨機分為輕度服藥亞組、輕度未服藥亞組;重度亞臨床甲減組患者隨機分為重度服藥亞組、重度未服藥亞組,每組30例。2個服藥組在常規(guī)治療基礎(chǔ)上加用左甲狀腺素片;2個未服藥組采用常規(guī)治療。記錄所有患者年齡、性別、BMI、血壓、血糖、血脂、甲狀腺功能等,比較患者內(nèi)脂素、內(nèi)皮素的差異。結(jié)果重度亞臨床甲減組、臨床甲減組內(nèi)脂素含量[(46.3±10.1)、(49.5±13.6)ng/m L]較對照組[(40.2±9.7)ng/m L]明顯升高(P0.05)。重度服藥亞組6個月時內(nèi)脂素較重度未服藥亞組升高[(42.9±6.4)ng/m L vs(39.3±5.5)ng/m L,P0.05]。結(jié)論相對于甲狀腺功能正常的腦梗死患者,重度亞臨床甲減合并腦梗死患者與內(nèi)脂素關(guān)系更密切。對腦梗死患者應積極篩查甲狀腺功能,及早發(fā)現(xiàn)和干預重度亞臨床甲減非常必要。
