中國南方地區(qū)中老年患者血清維生素D水平與骨密度的相關(guān)性

【摘要】:背景:維生素D是促進鈣吸收和維持人體骨骼系統(tǒng)健康的重要物質(zhì),但其血清水平與人體骨密度的關(guān)系仍存在爭議。目的:探索中國南方地區(qū)中老年患者血清維生素D水平與骨密度的相關(guān)性。方法:連續(xù)采集中國南方地區(qū)一個大型三甲醫(yī)院于2014年7月至2016年12月就診的50歲以上門診或住院患者的臨床數(shù)據(jù),分析患者的血清25-羥基維生素D[25(OH)D]水平與骨密度(腰椎、股骨頸和全髖)的關(guān)系。結(jié)果與結(jié)論:(1)納入對象資料分析:共納入1 154例患者,年齡(64.62±10.48)歲,女性占77.2%,平均血清25(OH)D水平為(62.07±37.40)nmol/L。血清25(OH)D隨著年齡增大逐漸降低,50-59歲、60-69歲和70歲以上患者25(OH)D不足/缺乏的比例分別為71.3%,77.0%和80.1%;(2)相關(guān)性分析顯示,總體患者的血清25(OH)D水平與腰椎骨密度無相關(guān)性,但與股骨頸和全髖骨密度存在弱正相關(guān)性(r=0.09,0.08,P0.01)。在維生素D缺乏[25(OH)D≤50 nmol/L]患者群中,血維生素D水平與腰椎骨、股骨頸和全髖骨密度呈顯著正相關(guān)(r=0.117,0.120,0.146,P0.01);(3)以性別做亞組分析,男性維生素D缺乏患者,其維生素D水平與腰椎、股骨頸和全髖骨密度都不存在相關(guān)性;女性維生素D缺乏患者,其維生素D水平與腰椎、股骨頸和全髖骨密度存在顯著正相關(guān)性(r=0.105,0.135,0.171,P0.01);(4)結(jié)果證實,中國南方地區(qū)中老年患者普遍存在維生素D不足或缺乏,但男性患者血清維生素D水平與骨密度無相關(guān),女性患者維生素D水平與骨密度正相關(guān)。
[Abstract]:Background: vitamin D is an important substance to promote calcium absorption and maintain the health of human skeletal system, but the relationship between serum level and bone mineral density is still controversial. Objective: to explore the correlation between serum vitamin D level and bone mineral density (BMD) in elderly patients in southern China. Methods: the clinical data of outpatients over 50 years old or hospitalized in a large third Class A hospital in southern China from July 2014 to December 2016 were collected. The relationship between serum 25-hydroxyvitamin D [25 (OH) D] and bone mineral density (lumbar vertebra, femoral neck and total hip) was analyzed.
