骨質(zhì)疏松患者下頜骨隨訪的初步研究

【摘要】:目的初步研究下頜體、下頜支做為骨密度隨訪指標(biāo)的臨床意義。方法采用美國Norland XR-800雙能X線骨密度儀,通過重復(fù)四次測定30例正常志愿者下頜骨骨密度,計算下頜體、下頜支重復(fù)性試驗精確度變異系數(shù)百分比;通過測定30例正常志愿者及45例骨質(zhì)疏松或骨量減少患者左股骨近端、正位腰椎、下頜骨骨密度,觀察其骨密度的差異;通過測定20例用藥患者6個月內(nèi)左股骨近端、正位腰椎、下頜骨骨密度,隨訪觀察股骨頸、Ward區(qū)、L2-4、下頜體、下頜支骨密度(BMD)隨時間的變化規(guī)律及臨床特點。結(jié)果 130例正常志愿者下頜體的重復(fù)性為2.08%,下頜支的重復(fù)性為2.75%,略高于Ward區(qū)的重復(fù)性變異系數(shù)2.27%。245例骨質(zhì)疏松或骨量減少患者下頜體、下頜支BMD顯著降低,與正常志愿者相比,差異有統(tǒng)計學(xué)意義(P0.05)。320例隨訪用藥患者下頜骨BMD變化先于其它部位出現(xiàn),下頜支BMD四周即可發(fā)生變化,早于下頜體。結(jié)論下頜骨骨密度變化敏感性高,非常適合做為骨質(zhì)疏松患者的臨床隨訪指標(biāo),優(yōu)于其他部位的骨密度測量。
[Abstract]:Objective to study the clinical significance of mandibular body and mandibular branch as bone mineral density (BMD) follow-up index. Methods the mandibular bone mineral density (BMD) of 30 normal volunteers was measured four times by Norland XR-800 dual energy X-ray absorptiometry, and the accuracy coefficient of variation percentage of mandibular body and mandibular branch reproducibility test was calculated. The bone mineral density (BMD) of the proximal end of the left femur, lumbar vertebrae and mandible in 30 normal volunteers and 45 patients with osteoporosis or osteopenia were measured, and the proximal end of the left femur was measured in 20 patients within 6 months. Bone mineral density (BMD) of anterior lumbar vertebrae and mandible were observed. The changes of bone mineral density (BMD) and clinical characteristics of L2-4, mandibular body and mandibular branch in the Ward area of femoral neck were observed. Results the reproducibility of mandibular body and mandibular branch was 2.08 and 2.75 in 130 normal volunteers, slightly higher than that in Ward area, the coefficient of variation of repeatability was 2.27.245 cases of osteoporosis or osteopenia. The BMD of mandibular Rami was significantly lower than that of normal volunteers. The difference was statistically significant (P0.05) .320 patients were followed up the changes of mandibular BMD appeared earlier than other parts of the mandible, the mandibular Rami BMD could change around four weeks, earlier than the mandibular body. Conclusion mandibular bone mineral density has high sensitivity and is suitable for clinical follow-up in patients with osteoporosis and is superior to other bone mineral density measurements.
