創(chuàng)面負(fù)壓治療對(duì)糖尿病足創(chuàng)面肉芽組織膠原蛋白沉積的影響

【摘要】:目的探討創(chuàng)面負(fù)壓治療(NPWT)對(duì)糖尿病足(DF)肉芽組織膠原蛋白沉積的影響。方法選取2014年1月至2016年6月在解放軍白求恩國(guó)際和平醫(yī)院內(nèi)分泌科住院的DF患者40例,隨機(jī)分為NPWT組和對(duì)照組(Con),每組各20例。NPWT組給予NPWT治療,Con組給予常規(guī)換藥治療。兩組分別于治療前及治療第14天取創(chuàng)面肉芽組織,行免疫組織化學(xué)染色,Western blot和RT-PCR分析Ⅰ、Ⅲ型膠原蛋白表達(dá)情況。結(jié)果免疫組織化學(xué)分析顯示,兩組治療后Ⅰ型、Ⅲ型膠原蛋白平均面密度值較治療前升高,NPWT組升高的程度高于Con組[(0.12±0.03)vs(0.34±0.04),(0.06±0.03)vs(0.19±0.03),P0.01]。Western blot分析顯示,兩組治療后Ⅰ型、Ⅲ型膠原蛋白相對(duì)蛋白表達(dá)值較治療前均升高,但NPWT組升高的程度高于Con組[(0.10±.042)vs(0.25±0.08),(0.11±0.03)vs(0.19±0.07),P0.01]。RT-PCR分析顯示,兩組治療后Ⅰ型、Ⅲ型膠原蛋白mRNA相對(duì)表達(dá)值較治療前均升高,但NPWT組升高程度高于Con組[(0.94±0.29)vs(3.24±0.54),(1.13±0.45)vs(3.13±1.20),P0.01]。結(jié)論 NPWT可促明顯進(jìn)創(chuàng)面肉芽組織Ⅰ、Ⅲ型膠原蛋白表達(dá)上調(diào),從而促進(jìn)創(chuàng)面的愈合。
[Abstract]:Objective to investigate the effect of wound negative pressure (NPWT) on the collagen deposition in the granulation tissue of diabetic foot (DF). Methods 40 patients with DF hospitalized in Department of Endocrinology, Heping Hospital, liberation army, from January 2014 to June 2016 were randomly divided into NPWT group and control group (Con). Each group was given NPWT treatment in each group of.NPWT, and Con group was given a regular basis. The two groups were treated with the wound granulation tissue before and fourteenth days of treatment. The immunohistochemical staining was performed in the two groups. The expression of type I and type III collagen was analyzed by Western blot and RT-PCR. Results the immunohistochemical analysis showed that after two groups of treatment, the surface density of type I and type III collagen egg was higher than that before the treatment, and the NPWT group increased. The degree was higher than that of Con Group [(0.12 + 0.03) vs (0.34 + 0.04), (0.06 + 0.03) vs (0.19 + 0.03), P0.01].Western blot analysis showed that the relative protein expression value of type I and type III collagen in the two group was higher than that before treatment, but the level of NPWT group was higher than that of Con Group [0.10 +.042) vs (0.25 + 0.08) and P0.01].RT-PCR analysis, P0.01].RT-PCR analysis. The results showed that the relative expression value of type I and type III collagen mRNA increased after treatment in the two groups, but the increase in NPWT group was higher than that of group Con [(0.94 + 0.29) vs (3.24 + 0.54), (1.13 + 0.45) vs (3.13 + 1.20). P0.01]. conclusion NPWT can promote the wound granulation tissue to be obviously up-regulated, thus promoting the healing of the wound.
