血清唾液酸與類風(fēng)濕關(guān)節(jié)炎相關(guān)性的研究

【摘要】:研究目的:通過測定唾液酸(SA)在類風(fēng)濕關(guān)節(jié)炎(RA)患者和健康對照組血清中水平并進(jìn)行對比,分析其與RA的實驗室檢查結(jié)果及臨床表現(xiàn)之間的相關(guān)性,進(jìn)一步探討SA在RA發(fā)病機(jī)制中的作用及臨床意義。研究方法:共收集120例RA患者和60例健康對照者。用酶法測定兩組血清SA水平并比較;將RA患者血清SA與其紅細(xì)胞沉降率(ESR)、C反應(yīng)蛋白(CRP)、類風(fēng)濕因子(RF)、抗環(huán)瓜氨酸肽抗體(抗CCP抗體)等檢查結(jié)果,以及病程、治療時間、關(guān)節(jié)腫脹計數(shù)(SJC)、關(guān)節(jié)疼痛計數(shù)(TJC)及疾病活動度評分(DAS-28)進(jìn)行相關(guān)性分析。統(tǒng)計學(xué)分析采用t檢驗、U檢驗、卡方檢驗及Pearson相關(guān)檢驗。結(jié)果:1.RA患者的血清SA水平(864.05±167.70)明顯高于健康對照組(570.99±67.37),差異具有統(tǒng)計學(xué)意義(P0.05)。2.RA患者血清SA水平與其CRP、ESR、RF、抗CCP抗體水平均呈正相關(guān),但只有與CRP和ESR的相關(guān)性具有統(tǒng)計學(xué)意義(r=0.53,P=0.00;r=0.52,P=0.00;r=0.06,P=0.55;r=0.01,P=0.89)。3.RA患者SA水平與年齡、TJC、SJC、DAS-28(CRP)、DAS-28(ESR)、Sharp評分均呈正相關(guān)(r=0.07,P=0.43;r=0.32,P=0.00;r=0.22,P=0.02;r=0.47,P=0.00;r=0.47,P=0.00;r=0.13,P=0.58),但只有與TJC、SJC、DAS-28(CRP)、DAS-28(ESR)的相關(guān)性具有統(tǒng)計學(xué)意義(P0.05);與病程、治療時間呈負(fù)相關(guān)性(r=-0.09,P=0.31;r=-0.06,P=0.51),差異無統(tǒng)計學(xué)意義(P0.05)。分析結(jié)果顯示:1.RA的疾病活動度與血清SA水平有關(guān)。2.蛋白質(zhì)糖基化末端SA的缺失可能與RA的發(fā)病有關(guān)。3.免疫球蛋白糖鏈的SA缺失可能誘發(fā)自身免疫反應(yīng),導(dǎo)致滑膜炎癥從而致病。4.聯(lián)合應(yīng)用Ig G0、Siglec-1、RF及抗CCP抗體有助于RA的早期診斷和治療效果的評價。5.血清SA水平可作為監(jiān)測RA病情變化、指導(dǎo)治療的臨床指標(biāo)。
[Abstract]:Objective: to determine the serum levels of sialic acid (SA) in patients with rheumatoid arthritis (RA) and healthy controls, and to analyze the correlation between sialic acid (SA) and the laboratory results and clinical manifestations of RA. Serum SA levels were measured by enzyme method in two groups, and the results of serum SA and erythrocyte sedimentation rate (ESR) / C reactive protein, (CRP), rheumatoid factor (RF), anti-cyclic citrullinated peptide antibody (anti-CCP antibody), course of disease, treatment time, etc.) of RA patients were compared. Joint swelling count (SJC), joint pain count (TJC) and disease activity score (DAS-28) were analyzed. T test, chi-square test and Pearson correlation test were used in statistical analysis. Results the serum SA level in RA patients (864.05 鹵167.70) was significantly higher than that in healthy controls (570.99 鹵67.37).
