【摘要】：目的探討碘超足量地區(qū)居民糖代謝紊亂與甲狀腺容積變化的關系。方法 2009年10月—2010年2月,采用分層整群抽樣方法,選取貴陽市社區(qū)居住10年及以上居民1 224例。研究對象均進行問卷調查、體格檢查、口服葡萄糖耐量試驗,采集靜脈血樣測定25(OH)D、甲狀旁腺激素(PTH)、促甲狀腺素(TSH)、尿酸、空腹血糖、餐后2 h血糖和空腹胰島素,采集尿樣測定尿碘,采用甲狀腺超聲測量甲狀腺各葉長度。按血糖水平分為正常人群、糖尿病前期人群及糖尿病人群。結果糖尿病人群甲狀腺容積(6.93±3.19)ml大于正常人群(6.29±2.15)ml(P=0.007)。甲狀腺容積與TSH(r=0.115,P0.01)、HOMA-IR(r=0.148,P0.01)、胰島素(r=0.138,P0.01)呈正相關。糖尿病前期及糖尿病是甲狀腺容積增大的獨立危險因素(P0.05)。結論糖代謝異�？赡苁怯绊懠谞钕偃莘e的獨立危險因素。
[Abstract]:Objective to study the relationship between the disorder of glucose metabolism and thyroid volume in the area of iodine excess. Methods from October 2009 to February 2010, 1 224 residents living in Guiyang for 10 years or more were selected by stratified cluster sampling. The subjects were investigated by questionnaire, physical examination, oral glucose tolerance test, venous blood samples were collected to determine 25 (OH) D, parathyroid hormone (PTH), thyrotropin (TSH), uric acid, fasting blood glucose, 2 h postprandial blood glucose and fasting insulin. Urine samples were collected to determine urinary iodine and thyroid ultrasound was used to measure the length of each lobe of thyroid gland. According to the level of blood sugar is divided into normal people, pre-diabetes and diabetes. Results the thyroid volume in diabetic patients (6.93 鹵3.19) ml was higher than that in normal subjects (6.29 鹵2.15) ml (P0. 007). The thyroid volume was positively correlated with TSH (r = 0.115, P 0.01), HOMA-IR (r = 0.148, P 0.01) and insulin (n = 0.138, P 0.01). Prediabetes and diabetes were independent risk factors for thyroid volume enlargement (P0.05). Conclusion abnormal glucose metabolism may be an independent risk factor affecting thyroid volume.
