【摘要】：目的探討老年2型糖尿病患者并發(fā)肺部感染的病原菌分布特征及其相關(guān)危險(xiǎn)因素。方法選取該院老年2型糖尿病并發(fā)肺部感染患者426例作為病例組和同期老年2型糖尿病未并發(fā)肺部感染患者430例作為對(duì)照組,采用法國(guó)生物梅里埃VITEK-60全自動(dòng)細(xì)菌分析儀分離和鑒定菌株,應(yīng)用多因素Logistic回歸分析老年2型糖尿病患者并發(fā)肺部感染的危險(xiǎn)因素。結(jié)果 426例肺部感染患者共分離出病原菌471株,以革蘭陰性菌(63.27%)和革蘭陽性菌(27.18%)為主。多因素logistic回歸分析顯示,年齡、侵襲性操作、FBG水平高、Alb水平低、APACHEⅡ評(píng)分高是肺部感染患者的獨(dú)立危險(xiǎn)因素(P0.05)。ROC曲線評(píng)價(jià)Logistic回歸模型預(yù)測(cè)效果的曲線下面積(AUC)及95%CI為0.912(0.853~0.986)。結(jié)論老年2型糖尿病患者并發(fā)肺部感染的病原菌分布較廣,影響肺部感染的危險(xiǎn)因素較多,需早期預(yù)防及控制,以降低患者發(fā)生感染的機(jī)率。
[Abstract]:Objective to investigate the distribution and risk factors of pathogenic bacteria in elderly type 2 diabetes mellitus complicated with pulmonary infection. Methods 426 elderly patients with type 2 diabetes mellitus complicated with pulmonary infection were selected as case group and 430 elderly patients with type 2 diabetes without pulmonary infection as control group. The strains were isolated and identified by VITEK-60 automatic bacterial analyzer. Multivariate Logistic regression analysis was used to analyze the risk factors of pulmonary infection in elderly patients with type 2 diabetes. Results A total of 471 strains of pathogenic bacteria were isolated from 426 patients with pulmonary infection, mainly Gram-negative bacteria (63.27) and Gram-positive bacteria (27.18).
