糖尿病視網(wǎng)膜病變患者全視網(wǎng)膜激光光凝前后視網(wǎng)膜動(dòng)、靜脈循環(huán)時(shí)間的改變

【摘要】:目的觀察糖尿病視網(wǎng)膜病變(diabetic retinopathy,DR)患者全視網(wǎng)膜激光光凝(pan-retinal laser photocoagulation,PRP)治療前后視網(wǎng)膜動(dòng)、靜脈循環(huán)時(shí)間的改變。方法經(jīng)熒光素眼底血管造影(fundus fluorescein angiography,FFA)檢查確診為重度非增殖期及增殖期DR且符合PRP適應(yīng)證的16例(16眼)患者納入研究,并行PRP。治療前、治療后1~3個(gè)月,采用FFA對(duì)所有患眼視網(wǎng)膜動(dòng)、靜脈循環(huán)時(shí)間進(jìn)行記錄,觀察視網(wǎng)膜動(dòng)脈4個(gè)分支主干出現(xiàn)顯影的時(shí)間、完全充盈的時(shí)間,視網(wǎng)膜靜脈4個(gè)分支主干全部出現(xiàn)層流的時(shí)間、完全充盈的時(shí)間,計(jì)算得出視網(wǎng)膜動(dòng)脈、視網(wǎng)膜毛細(xì)血管、視網(wǎng)膜靜脈、視網(wǎng)膜動(dòng)靜脈通過時(shí)間,對(duì)比觀察治療前后患眼不同時(shí)段的視網(wǎng)膜通過時(shí)間的變化。結(jié)果 DR患者PRP治療前視網(wǎng)膜毛細(xì)血管通過時(shí)間為(1.58±0.99)s,治療后為(2.19±1.23)s,差異有統(tǒng)計(jì)學(xué)意義(P=0.011)。DR患者PRP治療前后的視網(wǎng)膜動(dòng)脈、視網(wǎng)膜靜脈、視網(wǎng)膜動(dòng)靜脈通過時(shí)間的差異均無統(tǒng)計(jì)學(xué)意義(均為P0.05)。結(jié)論 PRP是DR患者的最重要治療方式,在PRP治療后1~3個(gè)月視網(wǎng)膜毛細(xì)血管通過時(shí)間的血流動(dòng)力學(xué)改變最為顯著,較PRP治療前有顯著延長。
[Abstract]:Objective to observe the changes of circulation time of retinal artery and vein in diabetic retinopathy (Dr) patients before and after treatment with pan-retinal laser photocoagulation. Methods 16 patients (16 eyes) with severe nonproliferative phase and proliferative phase Dr diagnosed by fundus fluorescein angiography were included in the study. Before and 1 ~ 3 months after treatment, FFA was used to record the circulation time of retinal artery and vein in all the affected eyes. The time of development of the main branches of the retinal artery and the time of complete filling were observed. The time of laminar flow in all four branches of retinal vein and the time of complete filling were calculated, and the passage time of retinal artery, retinal capillary, retinal vein, retinal artery and vein was calculated. The changes of retinal transit time before and after treatment were observed. Results the retinal capillary transit time before and after PRP treatment in Dr patients was 1.58 鹵0.99 s and 2.19 鹵1.23 s, respectively. The difference was statistically significant in retinal artery and retinal vein before and after PRP treatment in Dr patients. There was no significant difference in retinal arteriovenous transit time (P 0.05). Conclusion PRP is the most important treatment method for Dr patients. The hemodynamic changes of retinal capillary transit time in 1 ~ 3 months after PRP treatment are the most significant, which is significantly longer than that before PRP treatment.
