血清視黃醇結合蛋白-4與2型糖尿病及肥胖的相關性研究

【摘要】: [目的]研究不同糖耐量及體重人群中血清視黃醇結合蛋白-4(RBP4)水平的變化,并分析其與其它相關代謝和臨床生化指標的關系,從而進一步探討RBP4這個新近發(fā)現(xiàn)的又一脂肪因子與胰島素抵抗之間的相關性,以及其在2型糖尿病及肥胖發(fā)生發(fā)展中可能所起的作用。 [方法]采用酶聯(lián)免疫吸附法檢測13例正常糖調(diào)節(jié)正常體重者(NW-NGR)、14例正常糖調(diào)節(jié)超重/肥胖者(OW/OB-NGR)、26例2型糖尿病正常體重者(NW-T2DM)及34例2型糖尿病超重/肥胖者(OW/OB-T2DM)空腹狀態(tài)下血清視黃醇結合蛋白-4(RBP4)的水平;同時測定所有受試者的空腹血漿真胰島素(FINS)、空腹血糖(FBG)、糖化血紅蛋白(HBA1C)、總膽固醇(TC)、甘油三脂(TG)、高密度脂蛋白膽固醇(HDL-C)、低密度脂蛋白膽固醇(LDL-C)、收縮壓(SBP)、舒張壓(DBP)、身高、體重、腰圍(W)、臀圍(H),并計算腰臀比(WHR)、體重指數(shù)(BMI);以穩(wěn)態(tài)模型評估法計算胰島素抵抗指數(shù)(HOMA-IR):HOMA-IR=FBG×FINS/22.5。應用SPSS11.5for windows統(tǒng)計學軟件分析不同糖調(diào)節(jié)及體重人群血清RBP4水平的變化以及血清RBP4水平與這些臨床指標的相關性。 [結果] 2型糖尿病人群與糖調(diào)節(jié)正常人群之間血清RBP4水平差異無統(tǒng)計學意義[(1.05±0.51)vs(0.95±0.24)μmol / l,P=0.33];而各亞組間血清RBP4水平:OW/OB-T2DM組(1.20±0.34)μmol / l OW/OB-NGR組(1.08±0.27)μmol / l NW-T2DM組(0.86±0.06)μmol / l NW-NGR組(0.81±0.07)μmol / l。其中以OW/OB-T2DM組與OW/OB-NGR組的血清RBP4水平均明顯高于NW-NGR組及NW-T2DM組,差異均有統(tǒng)計學意義(P0.05或P0.01);但OW/OB-T2DM組與OW/OB-NGR組間,以及NW-T2DM與NW-NGR組間血清RBP4水平差異均無統(tǒng)計學意義。Pearson相關分析發(fā)現(xiàn)血清RBP4水平分別與性別(r=-0.280,P=0.009)、BMI(r=0.455,P0.001)、WHR(r=0.373,P0.001)、TG(r=0.223,P=0.038)、FINS(r=0.259,P=0.015)及HOMA-IR(r=0.415,P0.001)有顯著相關性。多元逐步回歸分析表明BMI(r~2=0.21,P0.001)、性別(r~2=0.17,P=0.045)是血清RBP4的獨立相關因素。 [結論]肥胖人群的空腹血清RBP4水平顯著升高;血清RBP4水平與性別、BMI、WHR、TG、FINS及HOMA-IR呈明顯相關性,而與空腹血糖及糖化血紅蛋白無顯著關聯(lián),提示其可能參與了肥胖癥、2型糖尿病等胰島素抵抗相關疾病的發(fā)病機制;RBP4可能成為又一治療2型糖尿病的作用靶點。
[Abstract]:[objective] to study the changes of serum retinol binding protein-4 (RBP4) levels in people with different glucose tolerance and body weight, and to analyze their relationship with other related metabolic and clinical biochemical indexes. The relationship between RBP4, another recently discovered fat factor, and insulin resistance, and its possible role in the development of type 2 diabetes and obesity were discussed. [methods] using enzyme-linked immunosorbent assay (Elisa) to detect OW / OB-T _ 2DM (n = 14) and OW / OB-T _ 2DM (n = 26) and OW / OB-T _ 2DM (n = 14), respectively. The level of serum retinol binding protein-4 (RBP4); Fasting plasma true insulin (FINSN), fasting blood glucose (FBGN), glycosylated hemoglobin (HbA1C), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), systolic blood pressure (SBP), diastolic blood pressure (DBP), height, weight, The waist circumference (WH), hip circumference (H), waist to hip ratio (WHR), body mass index (BMI) were calculated, and the homeostasis model assessment method was used to calculate the insulin resistance index (HOMA-IRN): HOMA-IRG FBG 脳 fins / 22.5. SPSS11.5for windows software was used to analyze the changes of serum RBP4 level and the correlation between serum RBP4 level and these clinical indexes in different glucose regulation and body weight groups. [results] there was no significant difference in serum RBP4 level between type 2 diabetes mellitus and normal glucose regulated subjects [1.05 鹵0.51)vs(0.95 鹵0.24 渭 mol / L], but the serum RBP4 level among subgroups was 1.20 鹵0.34) 渭 mol / l OW/OB-NGR (1.08 鹵0.27) 渭 mol / l NW-T2DM (0.86 鹵0.06) 渭 mol / l NW-NGR (0.81 鹵0.07) 渭 mol / L. The serum RBP4 levels in OW/OB-T2DM and OW/OB-NGR groups were significantly higher than those in NW-NGR and NW-T2DM groups (P 0.05 or P 0.01), but there was no significant difference between OW/OB-T2DM group and OW/OB-NGR group. There was no significant difference in serum RBP4 levels between NW-T2DM and NW-NGR groups. Pearson correlation analysis showed that the serum RBP4 levels were significantly correlated with BMIR 0.455T 0.455N 0.455P 0.001P 0.001 (P 0.001) and HOMA-IRR 0.259P0. 0015 (P 0. 001). Multivariate stepwise regression analysis showed that BMIR2 (0.21) and sex (0.17) P0. 045) were independent correlation factors of serum RBP4. [conclusion] the fasting serum RBP4 level was significantly increased in obese people, and the serum RBP4 level was significantly correlated with BMI-WHR, TGG fins and HOMA-IR, but had no significant correlation with fasting blood glucose and glycosylated hemoglobin. These results suggest that RBP4 may be involved in the pathogenesis of insulin resistance-related diseases such as obesity, type 2 diabetes and so on. RBP4 may be another target for the treatment of type 2 diabetes.
