【摘要】：目的:探討睪酮替代治療對糖尿病合并遲發(fā)性性腺功能減退患者胰島素抵抗的作用及其臨床療效。方法:82例糖尿病合并遲發(fā)性性腺功能減退患者隨機分為睪酮治療組(n=42)和對照組(n=40),兩組患者均維持原有降糖、調(diào)脂治療方案,治療組在此基礎(chǔ)上予十一酸睪酮膠丸口服,共治療6個月,觀察兩組治療前后體重指數(shù)、腰圍、血糖、血脂譜、胰島素敏感性、生殖激素以及中老年男性癥狀問卷(AMS)評估的相關(guān)癥狀評分及IIEF-5評分變化。結(jié)果:與治療前相比,治療組干預(yù)后體重指數(shù)(26.71±2.39 vs 25.15±2.28,P0.05)、腰圍(cm)(89.96±9.13 vs 85.03±9.58,P0.05)、糖化血紅蛋白(%)(7.73±1.31 vs 7.01±1.25,P0.05)、甘油三酯(mmol/L)(1.97±0.83 vs 1.41±0.69,P0.05)顯著下降,總睪酮(μmol/L)(7.16±2.21 vs 14.22±2.63,P0.05)顯著升高,穩(wěn)態(tài)模型評估的胰島素抵抗指數(shù)(3.76±1.18 vs 2.55±1.03,P0.05)和胰島素敏感指數(shù)(96±51 vs 138±53,P0.05)顯著改善,AMS心理和軀體評分顯著改善(P0.05),但IIEF-5評分(13.28±6.38 vs14.95±6.08,P0.05)改善不明顯。結(jié)論:睪酮替代治療可以改善糖尿病合并遲發(fā)性性腺功能減退患者胰島素抵抗并具有確切臨床療效。
[Abstract]:Objective: to investigate the effect of testosterone replacement therapy on insulin resistance in diabetic patients with delayed hypogonadism.Methods Eighty-two patients with diabetes mellitus complicated with delayed hypogonadism were randomly divided into two groups: testosterone group (n = 42) and control group (n = 40).Body mass index (BMI), waist circumference, blood glucose, blood lipid spectrum, insulin sensitivity, reproductive hormone and symptoms questionnaire of middle-aged and aged men were evaluated before and after treatment for 6 months.Results: compared with before treatment, the body mass index (BMI) of treatment group was 26.71 鹵2.39 vs 25.15 鹵2.28 P0.05, waist circumference was 89.96 鹵9.13 vs 85.03 鹵9.58, glycosylated hemoglobin was 7.73 鹵1.31 vs 7.01 鹵1.25P0.05, triglyceride was 1.97 鹵0.83 vs 1.41 鹵0.69P0.05, total testosterone (渭 mol/L)(7.16 鹵2.21 vs 14.22 鹵2.63 P0.05) was significantly increased.The insulin resistance index (ISI) and insulin sensitivity index (ISI) in steady-state model were 3.76 鹵1.18 vs 2.55 鹵1.03 P0.05) and insulin sensitivity index (96 鹵51 vs 138 鹵53 P0.05) significantly improved, but IIEF-5 score was 13.28 鹵6.38 vs14.95 鹵6.08 vs14.95 鹵6.08 P0.05).Conclusion: testosterone replacement therapy can improve insulin resistance in diabetic patients with delayed hypogonadism.
