富血小板血漿對糖尿病大鼠足潰瘍的影響

【摘要】:目的:探討富血小板血漿(PRP)對高血糖狀態(tài)下的糖尿病大鼠下肢皮膚潰瘍愈合速度的影響,并初步研究富血小板血漿治療糖尿病大鼠下肢皮膚潰瘍的機(jī)制。方法:糖尿病大鼠形成8周后下肢皮膚潰瘍造模,隨機(jī)分為治療組及對照組,創(chuàng)面分別給予富血小板凝膠或生理鹽水包裹治療。每天換藥并測量潰瘍面積,治療后第5d、14d采集血清及傷口組織,用ELISA法測血清及組織中MMP-2、MMP-9、TIMP-1、AGEs含量,傷口組織送病理進(jìn)行切片及HE染色觀察病理改變。結(jié)果:傷口造模后第5天治療組及對照組潰瘍面積無差異,PRP治療組組織中MMP-2、MMP-9含量均低于對照組,組織中TIMP-1含量高于對照組(P均0.05),HE染色新生血管數(shù)量多于對照組。治療后第14天PRP治療組潰瘍面積小于對照組(P0.05), MMP-2、MMP-9及TIMP-1差異較前更明顯(P0.01),HE染色PRP治療組成纖維細(xì)胞數(shù)量多于對照組,炎癥細(xì)胞浸潤量低于對照組。2組間AGEs含量及血中MMP-2、MMP-9、TIMP-1含量在各時(shí)間段均無差異。結(jié)論:1.利用燙傷的方法可以成功的制造糖尿病下肢皮膚潰瘍模型。2.富血小板血漿治療可能通過降低組織中MMP-2、MMP-9及提高TIMP-1含量,改善MMP-9/TIMP-1比值,促進(jìn)高血糖狀態(tài)下的糖尿病大鼠下肢皮膚潰瘍愈合。
[Abstract]:Objective: to investigate the effect of platelet-rich plasma (PRP) on the healing rate of skin ulcer of lower extremity in diabetic rats with hyperglycemia, and to study the mechanism of platelet rich plasma in treating skin ulcer of lower extremity of diabetic rats.Methods: diabetic rats were randomly divided into treatment group and control group after 8 weeks of formation of lower extremity skin ulcer. The wounds were treated with platelet-rich gel or normal saline respectively.The area of ulcer was measured every day. The serum and wound tissue were collected on the 5th day after treatment. The levels of MMP-2MMP 9 and TIMP-1AGEs in serum and tissue were measured by ELISA method. The pathological changes of wound tissue were observed by pathological section and HE staining.Results: there was no difference in ulcer area between the treatment group and the control group on the 5th day after wound modeling. The content of MMP-2 and MMP-9 in the treated group was lower than that in the control group, and the content of TIMP-1 in the tissue was higher than that in the control group. The number of neovascularization in the treated group was higher than that in the control group.On the 14th day after treatment, the area of ulcer in the PRP group was smaller than that in the control group (P 0.05), and the difference of MMP-2, MMP-9 and TIMP-1 was more obvious than that in the control group. The number of fibroblasts in the PRP treated with P0.01HE-HE staining was more than that in the control group.The amount of inflammatory cell infiltration was lower than that of control group. There was no difference in AGEs content and MMP-2, MMP-9 and TIMP-1 content in blood.Conclusion 1.The skin ulcer model of diabetic lower extremity can be made successfully by scalding method. 2.Platelet-rich plasma therapy may promote the healing of skin ulcer of lower extremities in diabetic rats with hyperglycemia by decreasing MMP-2 MMP-9 and increasing the content of TIMP-1 and improving the ratio of MMP-9/TIMP-1.
